TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Mr. Chairperson, Ladies and Gentlemen:
Pursuant to Rule XI-C of this Board, the Finance Committee has re-
viewed Miscellaneous Resolution #9018 and finds:
1) The program covers a period from June 1, 1979 through March 1, 1980.
2) Total cost for the program excluding in-house staff will be $11,110,
all of which will be incurred in 1979. The costs are $3,780 Print-
ing, $3,700 Postage, $2,000 Data Processing and $1,630 Office Supplies.
3) It is anticipated this program will generate total revenue of $11,110,
all of which will be realized in 1980.
4) The difference in timing between expenditures and revenues reflects
start-up cost and that revenues are on a reimbursement basis.
Pursuant to Rule XI-C of this Board, the Finance Committee finds $11,110
available in the Contingency fund; said funds are to be transferred to the
Health Division's Field Nursing Unit as follows:

The Finance Committee further recommends revenue from this program be
considered in the 1980 Budget.

IN RE: PREVENTION OF IMMUNIZABLE DISEASE - AGREEMENT BETWEEN THE MICHIGAN
DEPARTMENT OF PUBLIC HEALTH AND THE OAKLAND COUNTY HEALTH DIVISION
TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Mr. Chairperson, Ladies and Gentlemen:
WHEREAS the control and prevention of immunizable diseases is of primary
concern in Oakland County; and
WHEREAS there is a need to assess the immunization status of school
children in Oakland County as permitted under the proposed immunization status
regulation; and
WHEREAS a portion of the assessment effort will be funded by the
proposed agreement (attached) with the Michigan Department of Public Health; and
WHEREAS subject agreement covers a period from June 1, 1979 through
March 1, 1980.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners
authorize the agreement between the Michigan Department of Public Health and the
Oakland County Health Division for the prevention of immunizable diseases.
BE IT FURTHER RESOLVED that under the terms of this agreement the Michigan
Department of Public Health will reimburse the County of Oakland an amount not
to exceed twenty (20) cents per completed individual child immunization status
record for Fifth Grade students and thirty (30) cents per completed individual
child immunization status record for Tenth Grade students with total payment
not to exceed $11,110.

BUREAU OF DISEASE CONTROL & LABORATORY SERVICES
DIVISION OF IMMUNIZATION
AND
THE OAKLAND COUNTY HEALTH DIVISION
FOR THE PREVENTION OF IMMUNIZABLE DISEASES
This contract is entered into under authority granted by Act 368 of the
Public Acts of 1978.
PURPOSE
To allocate immunization grant funds in the amount indicated for the
reduction and eventual elimination of vaccine-preventable diseases
occurring in public and nonpublic schools.
OBJECTIVES/METHODOLOGY
A) Review and/or establish an immunization record for every child, grades
5 & 10, now attending a Michigan public or nonpublic school within
the health jurisdiction during the 1979-80 school year and annually
through the 1983-84 school year, plus Birmingham grades 1,2,4, & 6.
B) Establish a standard immunization schedule which will, if achieved,
assure that each child will be reasonably well protected against all
appropriate vaccine-preventable diseases throughout their remaining
school years.
C) Systematically identify, 1979-80 through 1983-84 school years, all
children needing one or several immunizations in order to achieve
the established standard.
D) Conduct follow-up activities designated to bring the immunization
levels of every student up to the stated criterion through an
individually tailored schedule of immunizations by March 1, 1984.
E) Submit all immunization status reports for grades 5 & 10, containing
the numbers of students by grade and school whose immunization histories
have been determined, to the MDPH by March 1, 1980. These forms will
be prepared based upon the SC613a Immunization Report Forms showing line
listing immunization levels of pupils in each school within the health
department's jurisdiction.

F) Prepare and submit billings to the Michigan Department of Public Health
for completed individual child immunization status records summarized on
SC613a forms.
PROGRAM REPORTING & EVALUATION
Progress reports on school immunization assessment and follow-up will be
submitted upon request. A written evaluation of the project will be submitted
no later than one month subsequent to the end of the agreement period. The
evaluation will present a brief historical review of the project and specific
accomplishments in relation to the objectives listed above.
PROGRAM BUDGET AND AGREEMENT AMOUNT
The Michigan Department of Public Health under the terms of this agreement will
reimburse the County of Oakland an amount not to exceed 20 cents per completed
individual child immunization status record for 5th grade students and 30 cents
per completed individual child immunization status record for 10th grade students.
The total payment shall not exceed $11,110 based upon total number of children,
grades 5 & 10, currently enrolled in public and nonpublic schools within the
jurisdiction of the County of Oakland. All forms must be legibly completed
to assure completion of a compliance audit.
REPORTING AND PAYMENT PROCEDURES
A Payment Voucher, form A-30, completed and signed by the contractor presenting
the total number of completed individual child immunization status records
placed on SC613a forms shall be prepared and submitted to the MDPH, Finance
Division, on a quarterly basis not later than 15 days after the close of the
last month of the quarter. The Payment Voucher shall be used by the MDPH to
reimburse the local health department for all completed status records under
the terms of the agreement. A final payment will be made approximately one
month after the completion of all forms and reports.
This agreement is conditionally approved subject to the availability of funds.
ASSURANCES
In compliance with,
1. Title VI of the Civil Rights Act of 1964, and the regulations
of the U.S. Department of Health, Education, and Welfare issued
thereunder, and,
2. the rules of the Michigan Civil Rights Commission,
the applicant agency assures that, in carrying out this program, no person shall
be excluded from participation, denied any benefits, or subjected to discrimination
on the basis of race, color, national origin, religion or sex (except where a
bona fide occupational qualification exists).

AGREEMENT PERIOD

by either party upon thirty days written notice to the other party or upon
the failure of either party to carry out the terms of the agreement. Any
changes to this agreement will be valid only if made in writing and accepted
by all parties to this agreement.
SPECIAL CERTIFICATION
The officer signing on behalf of the local agency certifies by his signature
that he is authorized to sign this agreement on behalf of the local agency;
that all terms of the agreement will be appropriately adhered to; and that
records and detailed documentation for the project or program identified in
this agreement will be maintained for a period of not less than three (3) years
from the date of the submission of the final expenditure report or until audit
findings have been resolved.
