HomeMy WebLinkAboutResolutions - 1985.05.23 - 11116May 9, 1985 MISCELLANEOUS RESOLUTION #8 5 15 2 BY: HEALTH AND HUMAN RESOURCES COMMITTEE, Nancy McConnell, Chairperson IN RE: COMMUNITY MENTAL HEALTH -- Residential Services Expansion for Developmentally Disabled Clients TO THE OAKLAND COUPTY ?,IRL, OF COMMISSIONERS: Mr. Chairperson, Ladies and Gentlemen: WHEREAS the Oakland County Ocy-,.,miy Mental Health Services Board has made requests to the Michigan Department of Mental Health to transfer fiscal responsibility for two foster care homes from Macomb-Oakland Regional Center, along with day programming and other ancillary costs for developmentally disabled residents of the homes as well as one Case Management Coordinator position to oversee services provided by the homes; and WHEREAS the Community Mental Health Services Board has also requested expansion funding from the State to support a placement out-of-county of one client who needs very specialized residential care not available in Oakland County; and WHEREAS Michigan law now requires that the County pay ten percent of the cost of operation of the two foster care homes so long as they are administered by the State, but the homes become a 100 percent State financial obligation when they are transferred to County Community Mental Health Services Board responsibility; and WHEREAS the Michigan Department of Mental Health has approved the foregoing requests in full, and has made sufficient funds available for their implementation in the amount of $179,472 for 1985, which requires County matching funds of $4,903; and WHEREAS the two foster care homes proposed for transfer to the Community Mental Health Services Board meet the satisfaction of the local units of government in the areas where the homes are located. NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners approve the proposals of the Community Mental Health Services Board to assume fiscal responsibility for two foster care homes and make one out-of-County placement of a developmentally disabled client, as further described in the appended narrative. BE IT FURTHER RESOLVED that matching funds authorized for these programs, in the amount of $4,903 for fiscal year 1985, and one additional Case Management Coordinator position be approved for the Community Mental Health Division to oversee the added residential services. CMH is proposing to transfer fiscal responsibility for the homes from the Macomb-Oakland Regional Center (MORC) along with responsibility for day programing and other ancil- lary services. Fourteen clients would be served by the two homes, and a new Coordinator would monitor their activities. The Michigan Department of Mental Health has approved this request and would provide funding for 90% of salary and fringe benefits of the new position. The County match portion would be 10%. Continued existence of the position would be contingent upon continued revenues from the State. COST OF NEW POSITION 1 Case Management Coordinator Salary @ 1 Year Step Fringe Benefits Annual Cost Six-month Cost $22,015 7,208 $29,223 $14,612 County Cost: Annual $ 2,922 Six-month 1,461 OAKLAND COUNTY COMMUNITY MENTAL HEALTH SERVICES BOARD i al S Reside vices ExPansi ilfn1.011222fILL11/2-isabi ed Clients Community Mental Health is proposing a program amendment to 1) transfer fiscal responsibility of two small group homes for developmentally dis- abled persons from Macomb Oakland Regional Center (MORO) along with the corresponding ancillary services including day programming, 2) add one case manager to provide program monitoring for the two homes, and 3) accept additional state funds to support an out-county placement of a county resident requiring specialized care. Focus Care Home This 6-bed licensed small group home for community residents is operated at 22170 Pontiac Trail in South Lyon. The home has been in operation since the beginning of 1984 and was funded principally with private funds donated by parents of the clients residing in the home. Due to the financial hard- ship imposed on the parents, the Department of Mental Health, through MORC, agreed to support the home with state funds on a temporary basis effective November 15, 1984. Since this home is for community residents without prior attachment to an institution, the Department encouraged our Board to assume the responsibility and made the necessary funding available. Negotiations took place with DMH to assure that funds transferred would be sufficient for home operations and all necessary support services including day programming and transportation. The CMH Board supported this transfer request based on those negotiations. The net residential cost of this home is $34.54 per day per client and provides 2,190 days of residential care. Southfield II This is an 8-bed licensed group home for developmentally disabled adults located in St. Bede's Convent at 18310 West Twelve Mile Road in Southfield and operated by Community Living Centers, Inc. This home has primarily been utilized by MORC for more medically fragile clients who have had linkage to a D.D. institution in the past. Both Community Living Centers and CMH support the use of this home for community demand clients as beds become available. Negotiations have taken place with MORC regarding this matter and they support the transfer. As in the Focus Care Home, sufficient funds would be transferred to CMH for ancillary and day program services in addition to the home contract budget. The net residential cost of this home is $56.00 per day per client and provides 2,920 days of residential care. Case Management Out County Placement CMH has current j71 responsibilities for this client. However, IDE . of the severity of 7ftipairment, the foster pare hts were not able to cc providing care. Although there are currently no vacancies available i land County capable of the client, a qualified home located lanti has a vacancy and is ,fliing to accept the placement. The DMH srts - this transfer and has made an additional $10,139 available to our Boa:- tc cover the incremental increase required to place the client in a small group home at the net cost of $37.69 per day. There is no County match required for this request. In order to monitor the activities of Focus Care and Southfield II, it is necessary to provide adequate case management for the 14 clients who be served by the two homes. We currently have four case managers plus a supervisor to monitor 155 clients. This represents a ratio of approximately 35 clients to each worker. A ratio of 25 to 1 is considered acceptable for this function. The addition of one case manager would enable us to serve the additional 14 clients plus reduce the client to worker ratio to approxi- mately 31 clients/worker. If this request is not supported, the CMH Board cannot support the transfer of the two troup homes. We hope to assume responsibility on July 1, 1985. The attached page reflects the budgetary implications for the balance of 1985 as well as the 1986 full year costs. As indicated by the budget summary, only the day programming, transportation and case management functions require County match.
