C•sling, Chairperson IN RE: TRANSFER OF STATE FUNDING TO COMMUNITY MENTAL HEALTH DIVISION FOR SELECTED COMMUNITY PLACEMENT FACILITIES TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS Mr. Chairperson, Ladies and Gentlemen: WHEREAS in Oakland County there are twenty (20) community placement facilities for developmentally disabled children and adults which are funded by the Michigan Department of Mental Health through its Macomb/Oakland Regional Center but which have historically been supervised and monitored by the County Community Mental Health Services Division; and WHEREAS the Community Mental Health Services Board, with the full support of the Department of Mental Health and Macomb/Oakland Regional Center, has requested that the funding for these twenty (20) facilities be transferred to the Community Mental Health budget together with funds for ancillary and day program services for the residents of the facilities; and WHEREAS consolidation of funding for the homes and supervision of the homes under the single authority of Community Mental Health would eliminate duplication of services and the need for so-called "paper admissions" of handicapped persons to Macomb/Oakland Regional Center in order for them to be placed in the twenty (20) foster care facilities; and WHEREAS consolidation of funding and supervision of the facilities would give County Community Mental Health staff complete authority over selection of residents for the facilities and over their in-home training and would give the foster home operators a single line of authority to follow; and WHEREAS transfer of funding for the twenty (20) facilities conforms with the goal set forth in Michigan's Mental Health Code to allow county Community Mental Health programs to assume responsibility for a greater share of public mental health services. NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners authorize the Community Mental Health Division to accept the transfer of full responsibility and associated State funds for the twenty(20) community placement facilities which are now supervised by Community Mental Health. Res. #83149, Transfer of State Funding to Community Mental Health Division for Selected Community Placement Facilities, reports with the recommendation that resolution be adopted with the following amendment: That local government approval must be received by Oakland County for the twenty (20) community placement facilities which are now supervised by Community Mental Health. Willia m Caddell, Chairperson OAKLAND COUNTY COMMUNITY MENTAL HEALTH SERVICES BOARD .TRANSFER OF RESPONSIBILITY FOR SELECTED COMMUNITY PLACEMENT FACILITIES Proposal The Community Mental Health Services Board is requesting transfer of funding to its budget for direct and ancillary costs of operation for twenty community placement facilities which have been supervised by Community Mental Health for many years; but are funded by the Macomb/Oakland Regional Center of the State Department of Mental Health. This transfer of full responsibility and funding is consistent with State law, Department of Mental Health policy, and the best interests of the present and future residents of the community placement facilities; the transfer is specifically endorsed by the Macomb/Oakland Regional Center. Residents of the homes are all developmentally disabled. Background - • _ In the early 1970's, Lapeer State Home and Training School began a joint project with Oakland County Community Mental Health to develop some community placement homes around the County for Oakland patients who had been living in the institution for years. Later, when the Macomb/Oakland Regional Center was established and our County was assigned to this State facility, the homes already opened were transferred to Macomb/Oakland Regional Center funding. Since then many other community placement homes have been developed by the State facility, totalling more than 190 in Oakland County alone. However, the earliest homes, opened by Lapeer and Community Mental Health, are a distinct group because historically they have been supervised by Community Mental Health Services rather than by the State institution. In fact, Community Mental Health staff provide all case management, supervision and oversight required for these 20 homes, and the State institution simply. channels the funds to the home operators, day programs and so forth, based on the authorization of the Community Mental Health personnel. Rationale The fact that the 20 community placement homes are supervised by .Community Mental Health, but receive their funding from the State institution, creates dual lines of responsibility for the home operators and confusion as to who is in charge. Also, this divided responsibility often necessitates duplication of efforts by CMH and institution staff. Consolidation of the operating funds for the homes with the supervising agency will give Oakland County total control over these community placement facilities, including selection of residents • for the homes. We will be able to do away with the need to obtain a "paper - admission" to Macomb/Oakland Regional Center in order to place a develop- mentally disabled person from the community into one of the 20 homes when, as an example, the natural parents are no longer capable of taking care of their handicapped child. This transfer of full.responsibility is in conformance, with the Michigan Mental Health Code and Department of Mental Health policy. The Code (Sec. 116) requires the Department to shift primary responsibility for services to the County CMH program 'whenever such-County shall have demonstrated a willingness and capacity to provide an adequate and appropriate system of mental health services to the citizens of such County." Specific Information The 20 community placement homes to be transferred are listed on the attached pages, with addresses, number of residents, and per-diem rates. There are 16 "Family Foster Care" facilities with a total of 48 beds, and 4 "Small Group" facilities with a total of 24 beds. All are "contract" homes because they receive mental health funds through contract, for in-home training of the residents, over and above the standard $14.86 per day paid by SSI (Federal funds) for room, board and basic supervision. The variation in rates paid to individual homes is due to the greater or lesser amounts of in-home programming which must be provided, depending on the needs of each develop- mentally disabled resident. The budget sheet which is also attached shows the cumulative Cost of these 72 beds for the final quarter of 1983, and for full-year 1984. The transfer of funding is proposed for October 1, 1983, the beginning of the State fiscal year. Ancillary and medical costs associated with the 72 developmentally disabled residents and the 20 homes, but not included in the homes' per-diem operating rates, would include medical and dental care for the residents as needed, when there is no other source of funding to cover these services. Also included are costs of wheelchairs and other specialized equipment needed by some of the residents; part-time Home Training Staff Aides who are assigned - occasionally to work one-on-one with some residents whohave especially demanding problems; medications not covered by other sources; and trans- portation of some residents to the day programs they attend. All these costs, plus the operating costs of the community placement homes themselves, would be a 100% State expense under the terms of Public Act 423 of 1981. The per- diem rates are fixed by contract with the home operators for the year, but ancillary costs are an estimate based on past history. If these costs exceed what has been budgeted to the Community Mental Health Board by the Department of Mental Health, the Board would secure a supplemental allocation from the Department to cover the remainder of the fiscal year. Fifteen of the 72 residents of the facilities attend New Horizons and Jewish Vocational Services day programs now, and the cost of this programming is included in the transfer proposal. This is the only cost which would recpAre County match, $9,135 on a Full-year basis at present rates. WILLIAM CADDELL, CHAIRPERSON IN RE: MISCELLANEOUS RESOLUTION 1183149-TRANSFER OF STATE FUNDING TO COMMUNITY MENTAL HEALTH DIVISION FOR SELECTED COMMUNITY PLACEMENT FACILITIES TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS Mr. Chairperson, Ladies and Gentlemen: Pursuant to Rule XI-C of this Board, the Finance Committee has reviewed Miscellaneous Resolution 583149 and finds the following: 1) The Oakland County Community Mental Health Division has requested the Michigan Department of Mental Health to transfer funds for twenty (20) community placement facilities, including funds for auxiliary and day program services, to their budget, 2) The Community mental Health Division staff has historically supervised and monitored these twenty (20) facilities in Oakland County which the Michigan Department of Mental Health has funded through the Macomb/ Oakland Regional Center,- 3) The consolidation of funding and supervision under the Community Mental - Health Division has been agreed upon by the Michigan Department of Mental Health and the Macomb/Oakland Regional Center, 4) The consolidation of funding and supervision under the Community Mental Health Division would eliminate duplication of services and provide one authority for the twenty (20) facilities, 5) The State has in the past considered funds transferred to the Community Mental Health Division as revenues returned to local units of government to meet the 41.6% Headlee requirement, 6) The proposed implementation date is October 1, 1983 with the State funding $209,014 and the County match being $2,254 for the three month period- October 1 through December 31, 1983 (per attached Schedule A), 7) Said County Match of $2,254 would be transferred from Contingency to the Community Mental Health Division as detailed below: From Contingency 10100-990-02-00-9990 To Community Mental Health 10100-166-01-00-3528 (2,254) 2,254 -0- FINANCE COMMITTEE Committee Vote: Motion carried on a roll call vote with Fortino passing and Page and Perinoff absent. ..... .....
