IN RE: VENEREAL DISEASE CONTROL GRANT RENEWAL ACCEPTANCE - 1981
TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Mr. Chairperson, Ladies and Gentlemen:
WHEREAS the Oakland County Board of Commissioners by Miscellaneous
Resolution 9615 dated November 20, 1980 authorized application for the 1981
venereal disease control grant; and
WHEREAS the grant is for the period of January 1, 1981 through December 31,
1981 and is in the amount of $48,000 or $17,810 less than the original grant
application amount of $65,810; and
WHEREAS this funding reduction necessitates the deletion of one (1)
Typist II position from the venereal disease control program; and
WHEREAS the Health Division has notified the State that certain previously
required program data will no longer be provided to the State but that we will make
every effort to maintain service delivery levels.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners
authorize the County Executive to accept the 1981 venereal disease control grant
in the amount of $48,000.
Mr. Chairperson, on behalf of the Health and Human Resources Committee, I
move the adoption of the foregoing resolution.

VI=EAL DISEASE GRANT PROPOSAL
January 1, 1980 - December. 31, 1921
- I. Problem:
Oakland County ranks fifth in Michigan for cases of gonorrhea and third for
cases of syphilis. Oakland is one of fifteen counties in the state defined as
"primary target areas" that account for 93% of the total gonolrhea cases and
90% of the total syphilis cases.
In 1978, reported gonorrhea cases in the state decreased from 1977 by 0.4%.
This vas the first decrease since 1963. Due to intensified casefinding efforts
that began in late 1978, there was a 6.1% increase in reported cases in the state .
for January through June, 1979.
In 1973, there was a 4% decrease in syphilis cases rate for the entire state.
In the first six months of 1979, an alarming reversal' occurred and an 11% in-
crease in the state occurred.
_ The residual damage of these two major infections have consequences beyond the
imTediate infection state. A majority of the venereal disease occurs in the
15-25 age group. Repeated Infections in known cases are between 10-25%. -
The young adult population is, therefore, at a high risk for such residual
consequences as chronic pelvici_nflammatory disease, sterility, and ectopic
. pregnancies (mmbar four cause in the maternal death rate in the United States).
Cakland County residents are at high risk of exposure to Venereal diseases and
its consequences.
II. Overview of Current Venereal Disease Program in Oakland County:
GOAL - To assure maintenance of tk a C '1 ad County Vers -J 1 Disease Control
Program that is designed to reduce tl incidence and pre - lance of venereal
diseases through interruption of disease transmission.
CaTEurIVES - The total venereal disease program serves Oakland County rcsidunts
and all visitors requesting this service by:
A. Providing quality diagnostic and treatment services for the venereal
diseases to all patients in need.
B. Thoroughly applying the epidemiologic process to all infectious or
• potentially infectious syphilis patients.

C. Providing ile:J:ediate appropriate follow-up of all persons reported as having
• a reactive serologic test for syphilis or positive darkfield examination.
D. Apolying the epideriologic process (interviewing and contact investigation
follow-up) to all roles and females diagnosed in the county health depart-
ment as being infected with gonorrhea.
E. Offering laboratory and follow-up services to private physicians and/or
community clinics as requested for gonorihea screening.
F. Totally imolementing the re-call for retesting program of males and females
treated for gonorrhea in the county health department and in other health-
care facilities participating in the female gonorrheatesting. program. Th
patients are to be retested by culture seven to ten (7-10) days following
.treaLwent and again four to six (4-6) weeks after the time of original treat-
ment.
G. Identifying roles and females infected with gonorrhea more than one time in
any given six-month period.- These individuals: will he classified as repeaters
.and highly intensive epidemiology will be applied to such cases.
H. Developing and raintaining a Gonococcal Pelvic inflailutatory Disease (PID)
management program.which will be:
1. Identifying females infected with gonococcal PID in venereal disease
clinics and hospital emergency rooms.
- 2. Ensuring that adequate treatment is provided these patients according
to the recommended tret.nent schedule of the Michigan Department of -
Public Health.
Providing post-treatment follow-up of these patients which will include
a test-of-cure culture examination 10-14 days following initiation of
treatment and a rescreening culture examination 4-6 weeks folic- ;-17
completion of treatment.
4. Providing for the application of intensive and rapid epidemiology
(interviewing and contact tracing) to these patients.
I. Establishing with private physicians the services of a contact referral and
follow-up system which will contribute to better case management of gonorle
patients diagnosed and treated in the private sector.
III. Specific Area Addressed by This Grant:
Cakland County Health Division requests that grant_ funds he used for the supportive
clerical and laboratory personnel needed to allow the professional team to do
quality treatment and follow-up.

IV. Objectives of This Grant:
Supplementing county resources provided as direct assistance in lieu of cash
(detailed in Section V of this agreement), the additional support rendered
through this agreement will enable the Oakland County program to maintain and
redirect activities and evaluation of same in the following areas:
A. To collect and summarize information from client records for local and.
state program analysis according to critical deadlines for each area.-
B. To provide clerical functions such as typing communication to various
community -resources and assist in finalizing materials for educational. -
information.

. C. To provide definitive laboratory testing services for our South Oakland
County Health Division clients as well as client specimens from various
other private and public ccitanunity health screening resources.
- V. .Metheedology: . .
. • A. Responsibilities of the Oakland County Health Division
1. Provide venereal disease diagnostic and treatment facilities which are
open to the public, accessible, and operative a sufficient amcunt of
time to meet the needs of the population.
2. . Staff the venereal disease clinic with personnel capable of providing
quality medical, nursing, epidemiologic, laboratory, and clerical. .
services for the patient in the most expeditious manner.
3. Provide laboratory facilities and laboratory expertise capable
.ing the county's gonorrhea testing program needs.
. 4. Provide immediate laboratory processing of on males
with _symptoms suggestive of gonorrhea seen d.7.1ing venereal disease
clinic hours, thus facilitating a diagnostic decision while the patien
is still in the clinic setting.
5. Submit to the Venereal Disease Division job descriptions for all posi-
tions supported under this agreement, and utilize these personnel only
to assist in the implementation and evaluation of the program methodology
described.
6. Prerore and submit records and reports required by the Venereal Cisease
Division on the forms provided and in accordance with a desigi -
schedule.

7. Submit ronthly, using Michigan Department of Public Health Form FIN-130,
a .request .for reimbursement of funds expended which are supported by
this agreement. Maintain and 1-1.ave available necessary records for audit
. by state or federal agencies for a period of not less than three years
or until a final audit has been completed.
B. Responsibilities of the Michigan Department of Public Health
1. Provide funds to cover expenditures during this agreement period accord-
ing to the attached budget pages (FIN Forms 110, 114, and 115), waYdrcazu
amount not to exceed $65,810.
2. Provide the following direct support in lieuof 'cash:
a. Assignment of two (2) public health field representatives to the
Oakland County Health Division.
b. Provide necessary forms, reports, and educational materials related
to the activities described.
c. Provide Thayer-Martin and/or tranSgrow culture r,edia required in
testing for gonorrhea.
d. Provide the primary recommended drugs utilized in the treatment of
venereal disease. -
VT. Evaluation Indicators:
- As related to the specific objectives stated in Section IV above, the following
indicators will assist evaluation:
A. Data collected will he received by Michigan DcT ,Tirt of Public H
Oakland County according to specified time intals.
B. Data collected will reflect, other state and national figures of
- variances in case rates for gonorrhea and syphilis.
C. Laboratory techniques will be checked according to CDC recommendations for
_ certification and licensure.
D. Laboratory testing will have rates of positive cases per total clieni
tion screened that con-pare closely to state and national rates.
VII. Program Budget and Agreement Amount:
The state share of the budget under the terms of this ayLeement may not exceed
$65,810. The total budget amount must he supported by a completed and si ,77),L,d

X. Agreement Period:
This agreement is in full fcrce and effect from January 1, 1981, to Decertiber
31, 1981.

to the other party or upon the failure of either party to carry out the terms
of the agreement. Any changes to this agreement %ill be valid only if made in
