IN RE: HEALTH DIVISION-1986/87 VENEREAL DISEASE REIMBURSEMENT AGREEMENT -
ACCEPTANCE
TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Mr. Chairman, Ladies and Gentlemen:
WHEREAS this agreement is for reimbursement of County expenditures
attributable to venereal disease prevention and control in Oakland County,
with funding made available by the Michigan Department of Public Health;
and
WHEREAS reimbursement is in the amount of $48,000 for the period.
October 1, 1986 thru September 30, 1987; and
WHEREAS these funds are incorporated in the 1987 Budget and General
Appropriations Act as Health Division revenue; and
WHEREAS this reimbursement provides the funding for two (2) positions
(Medical Technologist 02106 and Typist II 01394) in the Venereal Disease
Unit of the Health Division; and
WHEREAS the continuation of these two (2) positions is contingent upon
the continuation of the Venereal Disease Reimbursement Agreement; and
WHEREAS this agreement has been reviewed and approved as to form by
the Office of Corporation Counsel.
WHEREAS approval of the agreement does not obligate the County to any
future commitments.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of
Commissioners authorize the 1986/87 Venereal Disease Reimbursement
Agreement in the amount of $48,000.
BE IT FURTHER RESOLVED that the Chairman of the Oakland County Board
of Commissioners be and is hereby authorized to execute said agreement.
Mr. Chairman, on behalf of the Finance Committee, I move the adoption
of the foregoing resolution.

AGREEMENT BETWEEN THE
MICHIGAN DEPARTMENT OF PUBLIC HEALTH
hereinafter referred to as the "Department"
and
Oakland County Health Department
hereinafter referred to as the "Agency"

October 1, 1986 through September 30, 1987
I. Purpose_
To provide supplemental funding to the Agency Venereal Disease Program in
order to improve venereal disease patient-care, casefinding, prevention,
and educational services.
II. Objective
To continue to develop and maintain in the Agency a comprehensive venereal
disease control program designed to reduce the incidence and prevalence of
venereal disease.
III. Methodology
Supplemental state funding as well as state support provided as direct
assistance in lieu of cash (detailed in Section VI of this agreement), will
assist the Agency in developing, maintaining, and evaluating activities in the
following areas:
A. Venereal disease and other sexually transmitted disease diagnostic,
treatment, and patient-care services.
B. Syphilis case epidemiology.
C. Syphilis screening and positive-test follow-up.
D. Gonorrhea case epidemiology.
E. Gonorrhea screening and positive-test follow-up.
F. Venereal
reporting
G. Contact
disease case reporting and positive diagnostic test
from other private and public health-care providers.
referral systems with other health-care providers.
H. Professional and public information and educational programs.
I. Staff training.
IV. Program Agreement Amount
The Department under the terms of this agreement will provide funding not to
exceed $48,000.
All funds provided under this agreement must be used for the purposes stated in
this agreement and must not result in supplanting or reduction of local
effort. Any modification to this agreement must be made in writing and
executed by all parties to this agreement before the modification can be
implemented.
This agreement is conditionally approved subject to the availability of funds.
V. Responsibilities of the Agency
The Agency, in accordance with the general purposes and objectives of this
Agreement, will:
A. Meet or make reasonable progress toward attainment,of established program
standards of performance and objectives in the following areas:
1. Diagnostic, treatment, and patient-care services.
2. Syphilis casefinding, prevention, and management.
3. Gonorrhea casefinding, prevention, and management.
4. Information, education, and training.
These program standards of performance and objectives have been mutually agreed
upon by the Agency and the Venereal Disease Division of the Department. A
formal program review will be conducted annually with both parties
participating, to evaluate the degree of success achieved by the Agency in
meeting or making reasonable progress toward attainment of the established
program standards of performance and objectives. The result of this annual
program review will be provided in writing to the directors of both
participating agencies by the Venereal Disease Division of the Department within
ninety (90) days following the end of this agreement period.

B. Provide the Venereal Disease' Division' of the Department, through
designated channels, source information records and reports required for
program evaluation.
C. Provide the Venereal Disease Division of the Department the amount
of local and state cost-sharing funds to be used in support of venereal
disease program activities during this agreement period.
D. Provide the Venereal Disease Division of the Department budget
data on the expenditure of the $48,000 funded through this agreement i
only. This information should be prepared and submitted on the attached 2-
budget pages (FIN Forms • 110, 114, and 115).

E. Provide the necessary administrative, professional, and technical staff
for the operation of the program. 0
0 F. Utilize standard reporting forms prescribed by the Department.
G. Maintain adequate program and fiscal records and files including source
documentation to support program activities and expenditures made under
the terms of this agreement, as required.
H. Provide access to authorized representatives of the Department Federal
Grantor Agency, Comptroller General of the United States, or any of their
duly authorized representatives, to all records, files, and documentation
related to this agreement.
I. Submit examples of all proposed subcontracts to the Department as part of
the original plan and budget, for authorization under this master agreement.
Signed copies of subcontracts must be submitted within 30 days of
execution and will become attachments to this master agreement. The agency
furthermore shall:
1. require the contractor to comply with all applicable terms and
conditions of this master agreement. In the event of a conflict between
this master agreement and provisions of a subcontract, the provisions
of this master agreement shall prevail.
2. assume all responsibility for any work performed under a sub contract
including appropriate compliance with all terms and conditions of
the master agreement.
3. assure that any billing or request for reimbursement for
subcontract costs is supported by a valid subcontract and adequate
source documentation on costs and services.
J. Assure that all terms of the agreement will be appropriately adhered
to; and, that records and detailed documentation for the project or
program identified in this agreement will be maintained for a period of not
less than three (3) years from date of termination, the date of submission
of the final expenditure report or until audit findings have been
resolved.
K. Assure that all applicable federal and state laws, guidelines, rules, 0_
and regulations will be complied with in carrying out the terms of this
agreement including submission of a copy of any audit report related in Y
whole or part to this program.

L. Assure that all purchase transactions, whether negotiated or
advertised shall be conducted openly and competitively in accord with theg
principles and requirements of OMB Circular A-102 or A-110 as
applicable, and that records sufficient to document the significant
history of all purchases are maintained for a maximum of three years
after the end of the agreement period.
M. Inform the Department of any employee assigned to the Venereal Disease
Program who has retired from State of Michigan employment Acts 2 and 3 of
P.A. 1984 (Early Retirement Program). A monthly report shall be
required on the first of each month reporting the names of State early
retirants who performed work in the previous month in the Venereal Disease
Program under this agreement. Such reports are not required for any State
early retirant who reaches the age of 62 years.
VI. Responsibilities of the Department
A. Provide funds to supplement local resources during this agreement
period in an amount not to exceed $48,000, subject to the special
budgetary restriction provision indicated in Section IV of this agreement,
and based upon appropriate reports, records, and documentation maintained
by the Agency.
B. Provide the following direct support in lieu of cash:
1. Standard record and special report forms to be utilized in evaluation
of program activities.
2. Gonorrhea culture media required to meet established program
objectives.

8. Assignment of state and federal venereal disease program representatives
will be based on program need and available staff. The work schedule tq
be established by the Agency to best fit its program needs. 2
VII. Assurances
In compliance with
Title VI of the Civil Rights Act of 1964 and the Regulations of the U.S.
Department of Health and Human Services issued thereunder, and Section 504 of
the Rehabilitation Act of 1973, and The Michigan Handicappers' Civil Rights
Act (1976 P.A. 220), the Elliott-Larsen Civil Rights Act (1976 P.A. 453) and
the Rules of the Michigan Civil Rights Commission which have been
promulgated and adopted pursuant to the requirements of the Administrative
Procedure Act (1969 P.A. 306) as amended:
The Agency assures that, in carrying out this program, no person shall be excluded
from participation, denied any benefits, or subjected to discrimination on the
basis of race, creed, age, color, national origin, or ancestry, religion,
sex, or marital status (except where a bonafide occupational qualification
exists).
This policy of non-discrimination shall also apply to otherwise qualified
handicapped individuals.
VIII. Payment and Reporting Procedures
Payment shall be in equal quarterly installments to be paid on or about
the 15th of the first month of each quarter following execution of this
document. No payment shall be made until the agreement is fully executed.

7. Consultation, direction, and assistance in development of
venereal disease program and educational activities.
Final payment will be withheld until all required reports and documention are
received according to the terms of this agreement.
Reporting on the use of these funds shall be accomplished quarterly by
inclusion in the quarterly cost sharing expenditure reports (Financial
Status Report for Local Health Services, FIN-134).
IX. This agreement is in full force and effect from October 1, 1986 through September
30, 1987. This agreement may be terminated by either party by giving sixty
(60) days written notice to the other party stating the reasons for termination „,
and effective date or, upon the failure of either party to carry out the terms i
of the agreement, by giving ten (10) days written notice to the other party t
stating cause and effective date.

The Department may also terminate this agreement upon 15 days notice if the namj
of the Agency, or the name of the subcontractor, manufacturer, or supplier ofF,
the Agency subsequently appears in the register compiled by the
Michigan Department of Labor pursuant to Section 2 of Act 278 P.A. 1980. The act.
prohibits the State from entering into contract with certain employers who engage / 0 in unfair labor practices; to prohibit those employers from entering into i
certain contracts with others; to provide for the compilation and
distribution of a register of those employers and to provide for the
voiding of certain contracts.
Upon any such termination, any funds not authorized for use shall be returned
to the Department.
X. Amendments
Any changes to this agreement will be valid only if made in writing and accepted
by all parties to this agreement.
