WILLIAM CADDELL, CHAIRPERSON IN RE: VENEREAL DISEASE CONTROL GRANT ACCEPTANCE - OCTOBER 1, 1982 THROUGH JUNE 30, 1983 TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS Mr. Chairperson, Ladies and Gentlemen: WHEREAS Miscellaneous Resolution 48145 requires the Finance Committee to review acceptance of all grants that vary less than ten (10) percent from the original grant application; and WHEREAS Miscellaneous Resolution 482337 authorized the grant application to the Michigan Department of Public Health, Venereal Disease Division in the amount of $36,000 for the period October 1, 1982 through June 30, 1983; and WHEREAS the grant award is in the amount of $36,000, which is 100% funded from the State; and WHEREAS the grant award is for the period October 1, 1982 through September 30, 1983; and WHEREAS the Michigan Department of Public Health, Venereal Disease Division has indicated in the attached letter to anticipate an additional appropriation of $12,000 for the period July 1, 1983 through September 30, 1983; and WHEREAS application or acceptance of this grant does not obligate the County to any further commitment. NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners authorize and accept the Venereal Disease Control Grant in the amount of $36,000. BE IT FURTHER RESOLVED that the Chairperson of the Oakland County Board of Commissioners be and is hereby authorized to execute said grant contract. BE IT FURTHER RESOLVED that the Chairperson of this Board is hereby authorized to approve minor changes and grant extensions, not to exceed a ten percent variance, which are consistent with the grant as approved. BE IT FURTHER RESOLVED that this grant contract has been reviewed and approved as to form by the Office of Corporation Counsel. MR. Acceptance of 1982-83 Venereal Disease Program Agreement and Rudget Attached is the 1982-83 Venereal Disease Program Agreement and budget. Please start through the acceptance procedure as soon as possible. Please note that the application to submit was for S36,000 on a nine (9) month fiscal period. of 10/1/82 through 0/30/83. Telegraph Road Pontiac, Michigan 48053 Dear Doctor Locey: Enclosed is the original and threecopies of the venereal disease contractual agreement between the Oakland County Health Division and the Michigan Department of Public Health for the period October 1, 1982 through September 30, 1983 in the amount of $36,000. You will mote that this amount is 75 percent of your annual continuation level of $48,000 which is what I had previously advised you was the maximum we are able to commit at this time. You will also note that the contract period is for the total twelve month state fiscal year rather than the nine month (October 1982--June 1983) period I had originally indicated the contract period would cover. This change was a Departmental decision. By March, 1983, we will be in a position to advise you the exact amount of supplalent.al f!JndF; we will be able to provid.2 for the contract A: this point, I would not encoge you to initiate any cutbacks, but rather anticipate additional funds being made available by contract amendment. I would hope it will be the $12,000 additional funds necessary to bring the year's support to the continuation level. This will be dependent on the availability of funds (federal and state) which are still questionable as well as your venereal disease program's ability to meet the standards of performance agreed upon for fiscal year 1981-82. This will be determined through a total program review to be scheduled with you in January or February. The review will cover activity. for the period October 1, 1981 through September 30, 1982. We will also consider your utilization of contract funds in providing direct priority program services and the input of local resources for the venereal disease control program in Oakland County. Purpose To provide supplemental funding to the Oakland County Venereal Disease Program in order to improve venereal disease patient-care, casafinding, prevention, and educational services. • II. Objective To continue to develop and maintain in Oakland County a comprehensive venereal disease control program designed to reduce the incidence and prevalence of venereal disease. III. Methodology Supplemental state funding as well as state support provided as direct assistance in lieu of cash (detailed in Section V of this agreement), will assist the Oakland County program in developing, maintaining, and evaluating activities in the following areas: A. Venereal disease and other sexually transmitted disease diagnostic, treatment, and patient-care services B. Syphilis case epidemiology. C. Syphilis screening and positive-test follow-up. D. Gonorrhea case epidemiology. E. Gonorrhea screening and positive-test follow-up. F. Venereal disease case reporting and positive diagnostic test reporting from other private and public health-care providers. G. Contact referral systems with other health-care providers. H. Responsibilities of the Oakland County Health Divisi o n A. Meet or make reasonable progress tow ,,,--d attainment of established program standards of performance and objectives in the following areas: 1. Diagnostic, treatment, and patient—care services. 2. Syphilis casefinding, prevention, and management. 3. Gonorrhea casefinding, prevention, and management. 4. Information, education, and training. - These program standards of performance and objectives have been mutually agreed upon by the Oakland County Health Division and the Venereal Disease Division of the Michigan Department of Public Health. A formal program review will be conducted annually, with both parties participating, to evaluate the degree of success achieved by the Oakland County Health Division in meeting or making reasonable progress toward attainment of the established program standards of performance and objectives. The results of this annual program review will be provided in writing to the directors of both participating agencies by the Venereal Disease Division of the Michigan Department of Public Health within ninety (90) days following the end of this agreement period. B. Provide the Venereal Disease Division of the Michigan Department of Public Health, through designated channels, source information records and reports required for program evaluation. C. Provide the Venereal Disease Division of the Michigan Department of Public Health the amount of local and state cost—sharing funds to be used in support of venereal disease program activities during this agreement period. D. Maintain and have available necessary financial records for audits by state and federal agencies for a period of not less than three years or until a final audit has been completed. A. Provide funds to supplement local resources during this agreement period in an amount not to exceed $36,000, subject to the special budgetary restriction provision indicated in Section VI of this agreement. B. Provide the following direct support in lieu of cash: 1. Standard record and report folms to be utilized in evaluation of program activities. 2. Gonorrhea culture media required to meet established program objectives. 3. Primary recommended drugs utilized in the treatment of venereal disease. 2 4. Timely feedback of statistical and other data related to on-going program evaluation. 5. Available infotmation, education, training, and other program materials. 6. Assistance in meeting training needs of program personnel. 7. Consultation, direction, and assistance in development of venereal disease program and educational activities. 8. Assignment of two (2) state or federal venereal disease program representatives to the Oakland County venereal disease program. VI. Program Agreement Amount The state funding under the terms of this agreement may not exceed $36,000, which is available to cover the state funded cost of work performed through September 30, 1983. All funds provided under this agreement must be used for the purposes stated in this agreement and must not result in supplanting or reduction of local funding. Any modification to this agreement must be made in writing and executed by all parties to this agreement before the modification can be implemented. VII. Assurances In compliance with: A. Title VI of the Civil Rights Act of 1964 and Regulations of the U.S. Department of Health and Human Services issued thereunder, and B. The applicant agency assures that, in carrying out this program no person shall be excluded from participation, denied any benefits, or subjected to discrimination on the basis of race, creed, age, color, national origin or ancestry, religion, sex or marital status (except where a bonafide occupational qualification exists). This policy of nondiscrimination shall also applyto otherwise qualified handicapped individuals. .Fwportiq ad PaylnenL- ProceJdrc2s Reporting on the use of these funds shall be accomplishedquarterly by inclusion in the quarterly cost sharing expenditure reports (Financial Status Report for Local Health Services, FIN-134). Payment shall be in equal quarterly installments to be paid on or about the 15th of the first month of each quarter following execution of this document. No payment shall be made until the agreement is fully executed. Final payment will be withheld until all required reports and documentation are received according to the terms of this agreement-. Agreement Period This agreement is in full force and effect from October 1, 1982 to September 30, 1983, subject to the special budgetary restriction provision indicated in Section VI of this agreement. This agreement may be terminated by either party upon thirty-day written notice to the other party or upon the failure of either party to carry out the terms of the agreement. Any changes to this agreement will be valid only if made in writing. and signed by all parties to this agreement. This agreement is conditionally approved subject to the availability of funds. X. Special Certification The individual or officer signing on behalf of the local health department certifies by his signature that he is authorized to sign this agreement on behalf of the local agency; that all ferns of the . agreement will be appropriately adhered to; and that records and detail documentation for the project or program identified in this agreement will be maintained for a period of not less than three (3) years from the date of the submission of the final expenditure report or until audit findings have been resolved.
