Pruary 14, 1991 MISCELLANEOUS RESOLUTION # 91024 BY: PUBLIC SERVICES COMMITTEE, RUTH JOHNSON, CHAIRPERSON IN RE: DEPARTMENT OF PUBLIC SERVICES/CIRCUIT COURT PROBATION DIVISION-SUBSTANCE ABUSE SERVICES GRANT ACCEPTANCE TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS Mr. Chairperson, Ladies and Gentlemen: WHEREAS this Substance Abuse Services Grant (Probation Discretionary Enhancement Grant) has been approved by the Michigan Department of Corrections in the amount of $47,500; and WHEREAS the original application requested a funding level of $130,600 to cover the period October 1, 1990 through September 30, 1991; and WHEREAS the grant award for $47,500 represents approximately a sixty-four percent (64%) variance from the grant application of $130,600 as detailed in the attached schedule; and WHEREAS the award stipulates this funding level for the period of six (6) months only, effective October 1, 1990 through March 30, 1991; and WHEREAS funding to continue the program beyond the six (6) month period must go through another application process; and WHEREAS the grant award contract has been reviewed and approved as to legal sufficiency by the Office of Corporation Counsel; and WHEREAS this program is 100% state funded and does not obligate the County to further commitment. NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners accept the 1990-1991 Substance Abuse Services Grant (Probation Discretionary Enhancement Grant) in the amount of $47,500. BE IT FURTHER RESOLVED that the Chairperson of the Oakland County Board of Commissioners be authorized to execute this grant contract and be authorized to approve minor changes and grant extensions, not to exceed a fifteen percent (15%) variance, which are consistent with the approved grant. Prepared by Budget Division - LS Janauary 18, 1991 IMPACT STATEMENT - SUBSTANCE ABUSE SERVICES GRANT In September 1990 the Oakland County Circuit Court Probation Department requested a State Community Corrections grant of $130,600 to purchase substance abuse service for potent'al probation violators. The term of the grant would be October 1, 1990 to September 30, 1991 (state fiscal year). This was a request for renewal and increase of a previous grant of $96.000. The new grant would have expanded services. The State Board adopted a policy for fiscal 90/91 to renew grants for a period of only six months and in most cases entertain no requests for new grants or increases. Thus the State Board intended to grant Oakland County its previous 89/90 fund level for six months. We believe an error in math set the six months level at $47,500. In the previous years of this grant the State had funded this grant through the Oakland County's Probation Enhancement Board, which is now called the Oakland County Community Corrections Board. Under the guidelines of P.A. 511, the Circuit Court Probation Division and Oakland County must now apply for this same grant directly from the State in order to continue the program for providing substance abuse treatment services to probationers during their probation period. 4) This grant request covers the period of October 1, 1990 through September 30, 1991, with a funding level of $130,600. 5) This grant request reflects a variance of approximately thirty-six percent (36%) over the 1989/1990 grant award as detailed in the attached schedule. PUBLIC SERVICES COMMITTEE Informational Copies: Finance Committee , GRANT .REQUEST SUBSTANCE ABUSE SERVICES . SUMMARY The purpose of this program is to provide substance abuse treatment to probationers who continue drug use after being placed on probation. We are particularly interested in providing short term (21-28 day) in-patient treatment to indigent probationers. While both out patient and long term in-patient treatment are usually 'available to probationers, short term in-patient programs are generally filled with "private" pay (insured) substance abusers. Therefore indigent offenders are placed on a waiting list for admittance. Short term in-patient programs are particularly suited to _offenders who are currently in out- patient programs, or have participated in out-patient programs but continue substance abuse. The short term treatment programs are an alternative to probation violation and jail or prison sentences. They are also an alternative to placement in long term substance programs. These programs are available but are generally better suited to persons with a long history of both substance abuse and personal irresponsibility. They are more expensive and time consuming than needed for our target population. We propose hiring a part time substance abuse services coordinator to work with probation staff and the Oakland County Office of Substance Abuse. The Coordinator will screen referrals from probation staff and refer probationers to appropriate agencies for treatment. For our target poFulation the coordinator will refer the probationer to Oakland's Office of Substance Abuse for placement in an in-patient program. The Oakland OSA will contract with agencies and arrange placement and billing. The _USA is able to contract at a rate about half of the private pay rate. Oakland's Substance Abuse Office will also provide training for the coordinator. We propose funds for forty probationers in short term in-patient programs at an average of $3000 per placement. Our total $130,600 request includes a part time coordinator at $10,000 per year; $600 travel expense, and $120,000 for in-patient services. Evaluation will be via long term evaluation of (1) successful completion of substance abuse treatment, (2) successful completion of probation, and (3) further criminal arrests and convictions. I note this is a request for continuation of a probation enhancement grant which has been operational since March 1988. INSIGHT AT COLOMBIERE: - $6,720 for 28 day program; men and women age 18 and older; 54 beds - suburban clients are most often referred here - specializes in alcoholism but will treat associated abuse problems - accomplishes recovery through educational, individual, group and recreational therapy; strong emphasis on family, AA, and NA participation . . - develops an effective aftercare program to support and reinforce progress made during treatment - assures that recovery process incorporates home, social and work environments - stays in touch with individual's progress for two years following client's • stay . - Renewal Relapse Program (14 day - $230 per day) for those who have already been through inpatient treatment 2. TURNING POINT: - $6,300 for 28 day program; men and women age 18 and older - urban clients are most often referred here - 12-step spiritual program; clients transported to AA .and NA meetings located near their own homes - utilizes an holistic approach combined with educational, family, group, individual, and recreational therapy 3. OXFORD INSTITUTE: - $7,280 for 28 day program, $5,460 for 21 days; suburban men and women ace 18 and older; 60 beds - specializing in alcoholism and other drug addictions - holistic therapeutic approach - treatment modalities include education; individual, group and family therapy; nutritional and occupational therapy; recreational and spiritual counseling - outpatient evening recovery program $1,425; day recovery prooram $2,565 both include individual, famidy.and group therapy; didactic sessions •Erant Request - Substance Abuse Services 7 Page four 4. HOLLY GARDENS: - $4,620 for 28 day program; young suburban and urban men and women as well ' 25 adolescents; 68 beds (32 maximum for children age 14-17) - able to accommodate those adolescents stillattending school - combines disciplines of medical/psychiatric model with principles, practices and concepts of AA, NA and other self-help modalities for a family-style approach to treatment - oriented to assumption of personal responsibility - aftercare program; half-way house at $40 per day incorporating educational, family, individual and group therapy as well as extended treatment • Grant Request — Substance Abuse Services 7 Page five INTRODUCTION The grant applicant is the Oakland County Circuit Court Probation Department. The Probation Department provides services to Oakland's 1.1 million residents. More specifically, the Department prepares some 6000 pre-sentence investigation reports annually for Oakland's 14 judges. The Department also supervises the 5000 felons placed on probation who are residents of Oakland County. The Department is responsible for another 1500 probationers who live in other states and counties. . The Circuit Court Probation Department has been -in the forefront of developing and using community programs as alternatives to jail and prison. Among the programs utilized are community service, work release, probation residential centers, electronic monitoring, and various alcohol and drug rehabilitation programs. For some two and one-half years the Department has used state funds (from probation enhancement grants) to fund short term (21-28 day) substance abuse programs for probationers. The grant has been used to fund about 20 probationers per year. This is about half the population in need of the service. This grant application is a request to continue and increase funds for treatment. Z 5. . Gant Request - Substance Abuse Services - Page six PROBLEM STATEMENT Approximately 40% of Oakland's 5000 probationers are ordered as a condition of probation to submit to drug testing at the direction of the probation agent. Over 200 probationers are tested monthly. About 15% test positive. Most of these do not test positive a second time. However we estimate we have at least 20 offenders per month in need of substance abuse services. This is in addition to the approximately 1000 probationers already participating in substance abuse programs. Most of these probationers do not have insurance coverage so rely on publicly funded programs. Generally we can readily get probationers into out patient therapy or long term (6-12 month) in-patient programs. The problem is placing offenders in short term- twenty-one to twenty-eight day in-patient programs. These programs are ideally suited for persons who have been• participating in out-patient therapy but continue using, or persons who will not or can not maintain regular attendance at out-patient programs. However, there is generally a wait of one to two months for placement for indigent offenders. Our probationers are in need of immediate placement because (1) they are in violation of probation, and (2) drug or alcohol use is usually related to their criminal behavior. Over the past 30 months the Probation Department has used state Probation Enhancement funds ($95,000 year) to (1) hire a part time substance abuse coordinator to assist Probation agents in assessing offender needs and directing them to substance abuse agencies, and (2) place and fund probationers in programs, primarily short term in-patient programs. We have funded sixty-three probationers who have reported substance abuse or tested positive. 87% have successfully completed in-patient programs. 60% have managed to continue successfully on probation. 40% have had further problems that resulted in probation violation. • I •note that our funds are usually spent during the first six months Of the fiscal year. We estimate we could have placed twice as many as we have. I note that the average cost of a short term in-patient program has been $6500. We can cut this cost by over half by contracting through the County Substance Abuse Office. /1 — :Grant Request - Substance Abuse Services - Page seven PROGRAM OBJECTIVE Our objectives are: (I) Refer all probationers in need of substance abuse treatment to the appropriate agency or program via a substance abuse coordinator. (2) Provide funding for short term in-patient programs for select probationers who continue substance abuse. We estimate that approximately 200 probationers per year may be referred to service agencies with the assistance of a substance abuse coordinator. Further we estimate that about forty probationers per year may be placed in short . term in-patient programs. , Grant Request - Substance Abuse Services - Page eight METHODS (1) A part time (20 hours per week) substance abuse coordinator will work jointly with the Oakland County Substance Abuse Services Office and with the Probation Department. The coordinator will gain a knowledge of the services available in Oakland County. The coordinator will confer with probation staff to assist in assessment and placement of substance abusers. As in the past the Coordinator will be a student intern from Oakland University. The intern will be trained by both the Office of Substance Abuse and the Probation Department. (2) The coordinator will approve funding of substance abuse treatment for select probationers. These 'select probationers" are primarily those who continue substance abuse and have already participated in out-patient therapy. The Oakland County Office of Substance Abuse will contract with substance abuse service agencies for treatment for probationers. - • The Oakland County Substance Abuse Office presently contracts with six agencies that provide intensive out-patient or short term in-patient services. The cost of treatment ranges from $85 to $110 per day. I note this is about half the cost a private user would pay. We will use a figui-e of $3000 per probationer as the average cost of treatment. Procedurally, agents will refer probation substance abusers to the coordinator. The coordinator will evaluate the need for service. Those in need of funded services will be referred to the Office of Substance Abuse (OSA). USA will follow up with placement. Billing and payment will be handled by USA and Oakland County. Bills for services will come to USA who will forward this to County Accounting for payment with a copy to the coordinator. The coordinator will track expenses and provide feedback to agents. There will be quarterly reports of expenditures and services. Basically the evaluation involves a comparison of offenders who are funded and placed in substance abuse programs particularly suited to their needs with offenders who are forced to use ordinary resources available to indigent offenders. Probationers in the experimental and control groups are matched on the basis of some 13 characteristics. The project is intended to study the differences between the two groups in (1) successful completion of treatment, (2) successful completion of probation and (3) subsequent criminal arrests and convictions. Vital data is gathered by the substance abuse coordinator. Oakland University has agreed to commit a professor to assist in evaluation.
