THE INTERGOVERNMENTAL AGREEMENT BETWEEN THE COUNTY OF OAKLAND AND THE OAKLAND COUNTY COMMUNITY MENTAL HEALTH AUTHORITY FOR SUICIDE PREVENTION
SERVICES
To the Oakland County Board of Commissioners
Chairperson, Ladies and Gentlemen:
WHEREAS the Oakland County Community Mental Health Authority (CMH Authority) was awarded a five year
grant by the Michigan Department of Health and Human Services (MDHHS) titled "Transforming Youth Suicide
Prevention"; and
WHEREAS MR #15179 approved the intergovernmental agreement between the CMH Authority and the
Oakland County Health Division to address youth suicide prevention in Oakland County through training of key
community leaders from public health, schools, law enforcement, mental health and other community sectors
and implement suicide prevention programming for youth; and
WHEREAS the CMH Authority would like to amend the agreement to continue funding for the period beginning
October 1, 2015 through September 30, 2016; and
WHEREAS grant funding for the FY 2016 period is in the amount of $163,630, a decrease of $2,062 (1.2%)
which will not impact the special revenue position for this project; and
WHEREAS this amendment #1 will provide continued funding of the one (1) SR Full Time Eligible Public
Health Educator II position that was created in MR #15179; and
WHEREAS this amendment has been submitted through the County Executive's Contract Review Process
and is recommended for approval.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners accepts the
Oakland County Community Mental Health Authority's amendment #1 of the Intergovernmental Agreement
with the Oakland County Health Division for suicide prevention services in the amount of $163,630, for the
period October 1,2015 through September 30, 2016.
BE IT FURTHER RESOLVED that acceptance of this grant does not obligate the County to any future
commitment and continuation of the special revenue position in the grant is contingent upon continued future
levels of grant funding.
BE IT FURTHER RESOLVED that the Board Chairperson is authorized to execute the Grant Agreement and
approve changes and extensions not to exceed fifteen percent (15%).

AMENDMENT #1
The Parties, Oakland County ("County") and The Oakland County Community Mental Health
Authority ("CMH Authority"), agree and acknowledge that the purpose of this Amendment is to
modify as provided herein and otherwise continue the present contractual relationship between
the Parties as described in their current Suicide Prevention Services Contract, ("Contract").
In consideration of the extension of the mutual promises, representations, assurances,
agreements, and provisions in the Contract and this Amendment, the adequacy of which is
hereby acknowledged by the Parties, the Parties agree to amend the Contract as follows:
I. The Parties acknowledge and agree that the FY15 Exhibit I in the current Contract shall
be replaced with the FY16 Exhibit I as detailed below.
2. The Parties acknowledge and agree that the FY15 Exhibit II in the current Contract shall
be replaced with the FY16 Exhibit II as detailed below.
For and in consideration of the mutual assurances, promises, acknowledgments, warrants,
representations and agreements set forth in the Contract and this Amendment, and for other good
and valuable consideration, the receipt and adequacy of which is hereby acknowledged, the
undersigned hereby execute this Amendment on behalf of the CMH Authority and County and
by doing so legally obligate and bind the CMH Authority and County to the terms and conditions
of the Contract and this Amendment:

EXHIBIT I: Financial and Reporting Obligations
The funding made available under this contract is for the budget year October 1, 2015, through
September 30, 2016. The authorized amounts are as contained in the Financial Status Report as
detailed below.
The not-to-exceed amount of this agreement is $163,630. The source of funds is Federal Block
Grants for Substance Abuse and Mental Health Svc-Project of Regional and National
Significance and the CFDA number is 93.243. These funds are made available through the
Michigan Department of Community Health to the CMH Authority under federal grant award.
This Interlocal agreement is designated as a sub-recipient relationship and this is not a research
and development project.
The County will be reimbursed monthly for actual expenditures incurred after the submission,
review and approval by the CMH Authority of each expenditure report. The expenditure reports
must be submitted on a monthly basis no later than twenty (22) days after the close of each
calendar month.
A deviation allowance modifying an established budget category by $10,000.00 or 15 percent of
the funding for each project expenditure item, whichever is greater, is permissible. This
deviation allowance does not authorize new categories, subcontracts, equipment items, or
positions not shown in Financial Status Report.
The County shall submit to the CMH Authority quarterly reports on the progress on the key
Strategies and Activities outlined in Attachment A as applicable. The reports are due ten (10)
days after the end of each quarter.

Goals and Objectives
FY16
Goal I: Conduct a community needs assessment in Oakland County, using Qualitrics, to gain
information on the community's current strengths, areas for improvement, and goals for the
future.
Objective 1.1: Conduct the needs assessment among Oakland County school professionals.
Completion target 59 school staff members (297 public & private school principals, counselors,
and social workers surveyed with an estimated 20% response rate). We will encourage staff to
forward the survey to other staff, so responses may be higher. To be completed by 12/1/15.
• Develop needs assessment questionnaire.
• Distribute needs assessment to target population_
• Send follow up/reminders, as necessary, for individuals to complete needs assessment.
Objective 1.2: Conduct the needs assessment among healthcare professionals. Completion target
213 providers (116 substance use disorder providers, 184 emergency medicine providers, 666
primary care providers, 100 mental health providers surveyed with an estimated 20% response
rate). We will encourage healthcare staff to forward the survey to other providers, so responses
may be higher. To be completed by 12/1/15.
• Develop needs assessment questionnaire.
• Distribute needs assessment to target population.
• Send follow up/reminders, as necessary, for individuals to complete needs assessment.
Objective 1.3: Conduct the needs assessment among Jaw enforcement professionals.
Completion target 16 law enforcement officers (83 law enforcement officers surveyed with an
estimated 20% response rate). We will encourage law enforcement staff to forward the survey to
other staff, so responses may be higher. To be completed by 12/1/15.
• Develop needs assessment questionnaire.
• Distribute needs assessment to target population.
• Send follow up/reminders, as necessary, for individuals to complete needs assessment.
Objective 1.4: Analyze data received through the needs assessment, to begin 1/1/16. Data will
be available to use to inform suicide prevention activities through FY16.

• Hand score qualitative data.
• Score quantitative data using the Qua'tries program.
• Review and summarize findings from needs assessment.
Goal 2: Increase skills in the treatment and/or referrals of youth at risk for suicide to
community agencies or private providers, with an emphasis on continuity of care.
Objective 2.1: Coordinate and host QPR (Question, Persuade, Refer) training for the Substance
Use Disorder network in Oakland County. Expected outcome, 25% of SUD staff will complete
QPR training, using training log/evaluations to measure.
• Provide SUD staff with QPR log in information and evaluation tools, completed by
12/1/15.
• Review QPR reports and send reminders for staff to complete training by 1/1/16.
• Review evaluation tools received and send reminders for staff to complete evaluation by
1/8/16.
Objective 2.2: Implementation of suicide prevention strategies will begin with middle and high
schools in Waterford School District middle and high schools during the 2015/2016 school year.
Expected outcome, 40% of Waterford staff and 35% of Waterford students will complete SOS
training, using training log/evaluations to measure.
• Continue joint planning with Waterford school administration and Waterford Coalition
for Youth to determine readiness and finalize implementation plan for youth suicide
prevention activities in the school.
• Alert Common Ground and private providers in the Waterford area when suicide
prevention activities will occur, and determine that adequate capacity is present to
provide follow-up care to youth who may need evaluation by a mental health provider.
• Provide resource list to Waterford School District identifying public and private mental
health and substance use disorder practitioners in Waterford area who are accepting new
patients.
• Engage Waterford counseling and social work staff in discussion regarding preparedness
to respond to students needing follow-up during or after suicide prevention activities.
Information obtained will be used to inform decisions about further trainings/education
needed.
• Train Waterford middle school and high school staff in SOS (signs of suicide) and collect
completed evaluation tool.

• Train parents of Waterford middle school and high school students in SOS (signs of
suicide)and collect completed evaluation tool.
• Train Waterford middle school and high school students in SOS (signs of suicide) during
academic center class period and collect completed evaluation tool. Obtain active
consent for youth under age 18.
• Complete Safe TALK training for Waterford middle school and high school staff.
• Identify Waterford staff, parents, and youth who may be interested in participating in
further community suicide prevention activities. Staff, parents, and youth will be
identified through participation in SOS and suicide prevention activities that occur at
Waterford for further activities such as a youth task force, working with coalitions, and
forum participation.
• Rochester will be the alternate district, if needed, in above activities if Waterford is
unable to participate in activities during the 2015/2016 school year.
Objective 2.3: Implementation of suicide prevention strategies will begin with Oakland
University (OU), during the 2016/2017 school year. Expected outcome, 10 staff, 20 parents,
and 25 students will complete the SOS training in FY16. Measured by training logs/evaluations.
• Engage in discussion with OU school administration, William Beaumont School of
Medicine, GRASP program, and Rochester/Auburn Hills Community Coalition to
identify needs related to youth and young adult suicide prevention.
• Alert Common Ground and private providers in the Rochester area when suicide
prevention activities will occur, and determine that adequate capacity is present to
provide care to youth who may need evaluation by a mental health provider.
• Provide resource list to Oakland University identifying public and private mental health
or substance use disorder practitioners in Waterford area who are accepting new patients.
• Engage OU Counseling Center and SEHS Counseling Center in discussion regarding
readiness to respond to students who might need follow-up during or after suicide
prevention activities. Information obtained will be used to inform decisions about
further trainings/education needed.
• Train OU staff in SOS (signs of suicide) and collect completed evaluation tool.
• Train parents of OU students in SOS (signs of suicide) and offer during new
student/freshman orientation in summer of 2016 and collect completed evaluation tool.

• Train OU students in SOS (signs of suicide) and collect completed evaluation tool.
Obtain active consent for students under age 18.
• Offer trainings for medical students such as QPR, ASIST, Safe TALK, etc. Coordination
with medical school to determine dates/times and how to reach maximum attendance
will occur via meetings, email, etc.
• Identify OU staff, parents, and students who may be interested in participating in further
community suicide prevention activities. Staff, parents, and students will be identified
through participation in SOS and suicide prevention activities that occur at OU for
further activities such as a the task force, working with coalitions, and forum
participation.
Objective 2.4: Coordinate and implement minimally 1 AMSR (Assessing and Managing Suicide
Risk) for core provider agency clinical staff of Oakland County Community Mental Health to
attend. Expected outcome, 50 clinical staff will be trained in the AMSR over FY16, to be
measured by training logs and evaluations.
• Coordinate and secure trainer, date and time, location, small fee associated for
participants, advertising, etc.
• Training will occur and be completed by 4/1/16.
• Collect completed evaluation tools.
Objective 2.5: Host two community forums (one for youth in the fall of 2015, which will focus
on promoting youth voice and giving the opportunity in small breakout sessions for youth to
identify needs related to suicide prevention. Also, one for community members in the spring of
2016, which will focus on promoting knowledge and understanding of how to access community
resources related to suicide prevention) in partnership with the Youth Suicide Prevention Task
Force in Oakland County. Expected outcome, attendance of 25 youth at fall forum and
attendance of 25 community members at spring forum. Measured by attendance log/evaluation.
• Partner with Oakland County ISD to obtain optimal youth participation. This will occur
through joint meetings between the two grants, Community Management Team meetings,
and task force meetings and having forums as an agenda item at these various meetings.
• Coordinate date and time, location, agenda, and advertising for forums.
• Gather information from forum participants through small group discussion and written
feedback on youth suicide prevention in Oakland County.
• Review/discuss information received at forums with the task force and use it to inform
youth suicide prevention work in Oakland County.

Objective 2.6: Research best practices in means restriction and strategize how to apply these
practices in Oakland County. Expected outcome, a best practice will be identified by Oakland
County to use in means prevention training, to be measured by implementation of the best
practice.
• Identify groups/programs, such as prescription drug program, in Oakland County that are
available and assist in means restriction activities. Once identified utilize already
existing programs to link community members with for means restriction information.
• Review state and national programs utilized and evidenced based practices that are
available. Research what programs exist and take information to task force and TSYP

Communities regarding coordination of coalitions with the TYSP grant and task force, to
be completed by 12/1/16.
Objective 3.5: Begin building relationships with healthcare providers in Oakland County to
increase opportunities for training and collaboration around suicide prevention. Expected
outcome, TYSP staff will attend community meetings (see below activities) with healthcare
providers, minimally quarterly to provide information on grant activities/opportunities for

TYSP grantees will review resources for Zero Suicide online and will bring those
resources back to their respective agencies to begin implementing these activities by
1/1/16.

and OCHD will occur by 1/1/16. Information from this assessment will be incorporated
into activities at OCCMHA and OCHD.
• Oakland County will propose sending two teams to the Zero Suicide Academy, to
implement the Zero Suicide approach on a county-wide level. If selected, Oakland
County will attend the Zero Suicide Academy in the summer of 2016.
Objective 3.7: The TSYP grantees will attend the SAMHSA national grant meeting in
Washington DC in FY16 to meet and collaborate with other grantees nationally. Expected
measure will be TYSP attendance and representative from Oakland County.
Objective 3.8: Oakland County Community Mental Health Authority and Oakland County
Health Division will send a team of grantees to the Annual Association of Suicidology
Conference in Chicago in spring of FY16. Expected outcome to increase knowledge and skill in
the area of strategies, trainings, and implementation related to youth suicide prevention of TYSP
grantees that can be brought back to Oakland County. Measure will be attendance.

EXHIBIT I: Financial and Reporting Obligations
The funding made available under this contract is for the budget year October 1, 2015, through
September 30, 2016. The authorized amounts are as contained in the Financial Status Report as
detailed below.
The not-to-exceed amount of this agreement is $163,630. The source of funds is Federal Block
Grants for Substance Abuse and Mental Health Svc-Project of Regional and National
Significance and the CFDA number is 93.243. These funds are made available through the
Michigan Department of Community Health to the CMH Authority under federal grant award.
This Interlocal agreement is designated as a sub-recipient relationship and this is not a research
and development project.
The County will be reimbursed monthly for actual expenditures incurred after the submission,
review and approval by the CMH Authority of each expenditure report. The expenditure reports
must be submitted on a monthly basis no later than twenty (22) days after the close of each
calendar month.
A deviation allowance modifying an established budget category by $10,000.00 or 15 percent of
the funding for each project expenditure item, whichever is greater, is permissible. This
deviation allowance does not authorize new categories, subcontracts, equipment items, or
positions not shown in Financial Status Report.
The County shall submit to the CMH Authority quarterly reports on the progress on the key
Strategies and Activities outlined in Attachment A as applicable. The reports are due ten (10)
days after the end of each quarter.

• Train Waterford middle school and high school staff in SOS (signs of suicide) and collect
completed evaluation tool.
• Train parents of Waterford middle school and high school students in SOS (signs of
suicide)and collect completed evaluation tool.
• Train Waterford middle school and high school students in SOS (signs of suicide) during
academic center class period and collect completed evaluation tool. Obtain active
consent for youth under age 18.
• Complete Safe TALK training for Waterford middle school and high school staff.
• Identify Waterford staff, parents, and youth who may be interested in participating in

• Rochester will be the alternate district, if needed, in above activities if Waterford is

University (OU), during the 2016/2017 school year. Expected outcome, 10 staff, 20 parents,
and 25 students will complete the SOS training in FY16. Measured by training logs/evaluations.
• Engage in discussion with OU school administration, William Beaumont School of
Medicine, GRASP program, and Rochester/Auburn Hills Community Coalition to identify
needs related to youth and young adult suicide prevention.
• Alert Common Ground and private providers in the Rochester area when suicide
prevention activities will occur, and determine that adequate capacity is present to
provide care to youth who may need evaluation by a mental health provider.
• Provide resource list to Oakland University identifying public and private mental health
or substance use disorder practitioners in Waterford area who are accepting new
patients.
• Engage OU Counseling Center and SEHS Counseling Center in discussion regarding
readiness to respond to students who might need follow-up during or after suicide
prevention activities. Information obtained will be used to inform decisions about
further trainings/education needed.

• Train parents of OU students in SOS (signs of suicide) and offer during new
student/freshman orientation in summer of 2016 and collect completed evaluation

Risk) for core provider agency clinical staff of Oakland County Community Mental Health to
attend. Expected outcome, 50 clinical staff will be trained in the AMSR over FY16, to be
measured by training logs and evaluations.
• Coordinate and secure trainer, date and time, location, small fee associated for
participants, advertising, etc.
• Training will occur and be completed by 4/1/16.
• Collect completed evaluation tools.
Objective 2.5: Host two community forums (one for youth in the fall of 2015, which will focus
on promoting youth voice and giving the opportunity in small breakout sessions for youth to
identify needs related to suicide prevention. Also, one for community members in the spring

community resources related to suicide prevention) in partnership with the Youth Suicide
Prevention Task Force in Oakland County. Expected outcome, attendance of 25 youth at fall
forum and attendance of 25 community members at spring forum. Measured by attendance
log/evaluation.
• Partner with Oakland County ISD to obtain optimal youth participation. This will occur
through joint meetings between the two grants, Community Management Team
meetings, and task force meetings and having forums as an agenda item at these
various meetings.
• Coordinate date and time, location, agenda, and advertising for forums.
• Gather information from forum participants through small group discussion and written
feedback on youth suicide prevention in Oakland County.
• Review/discuss information received at forums with the task force and use it to inform

Pursuant to Rule XII-C of this Board, the Finance Committee has reviewed the above referenced resolution
and finds:
1. The Oakland County Community Mental Health Authority (CMH Authority) has awarded the Oakland
County Department of Health and Human Services/Health Division funding in the amount of
$163,630; a decrease of $2,062 from the previous award.
2. Funds will be used to train key community leaders from schools, law enforcement, the community as
well as youth and implement suicide prevention programming for youth.
3. Miscellaneous Resolution #15179 approved an intergovernmental agreement between CMH Authority
and Oakland County Health Division to address youth suicide prevention through September 30, 2015
4. CMH Authority would like to amend the agreement to continue funding for the period beginning
October 1, 2015 through September 30, 2016.
5. Under this project, a portion of the funding will be used to cover one (1) Special Revenue Full-Time
Eligible Public Health Educator II position (#1060233-11673) within the Health/CHPIS unit of the
Health Division under the Suicide Prevention Fund (#28615).
6. The future level of service, including personnel, will be contingent upon the level of funding available.
