IN RE: DEPARTMENT OF HEALTH AND HUMAN SERVICES/HEALTH DIVISION — AMENDMENT #2 OF
THE INTERLOCAL AGREEMENT BETWEEN THE COUNTY OF OAKLAND AND THE OAKLAND
COUNTY COMMUNITY MENTAL HEALTH AUTHORITY FOR SUICIDE PREVENTION SERVICES
To the Oakland County Board of Commissioners
Chairperson, Ladies and Gentlemen:
WHEREAS the Oakland County Community Mental Health Authority (CMH Authority) was awarded a five year
grant by the Michigan Department of Health and Human Services (MDHHS) titled "Transforming Youth Suicide
Prevention"; and
WHEREAS Miscellaneous Resolution (MR) #15179 approved the interlocal agreement for fiscal year (FY)
2015 between the CMH Authority and the Oakland County Health Division to address youth suicide prevention
in Oakland County through training of key community leaders from public health, schools, law enforcement,
mental health and other community sectors and implement suicide prevention programming for youth; and
WHEREAS MR#16004 approved amendment #1 to the interlocal agreement between the CMH Authority and
the Oakland County Health Division to include additional funding and extend the grant period for fiscal year
(FY) 2016 ending September 30, 2016; and
WHEREAS the CMH Authority is issuing amendment #2 to increase funding from $163,630 to $220,979, an
increase of $57,349 (35%), for the period beginning October 1,2015 through September 30, 2016; and
WHEREAS this amendment continues funding of one (1) Special Revenue (SR) Full Time Eligible Public
Health Educator II position that was created in MR #15179; and
WHEREAS this amendment has completed the Grant Review Process according to the Board of
Commissioners Grant Procedures and is recommended for approval.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners accepts the
Oakland County Community Mental Health Authority's amendment #2 of the Interlocal Agreement with the
Oakland County Health Division for suicide prevention services to increase funding from $163,630 to
$220,979, an increase of $57,349 (35%) for the period October 1,2015 through September 30, 2016.
BE IT FURTHER RESOLVED that acceptance of this amendment does not obligate the County to any future
commitment and continuation of the special revenue position in the grant is contingent upon continued future
levels of grant funding.
BE IT FURTHER RESOLVED that the Board Chairperson is authorized to execute the Grant Agreement and
approve changes and extensions not to exceed fifteen percent (15%).

The Parties, Oakland County ("County") and The Oakland County Community Mental Health
Authority ("CMH Authority"), agree and acknowledge that the purpose of this Amendment is to
modify as provided herein and otherwise continue the present contractual relationship between the
Parties as described in their current Suicide Prevention Services Contract, ("Contract").
In consideration of the mutual promises, representations, assurances, agreements, and provisions
in the Contract and this Amendment, the adequacy of which is hereby acknowledged by the
Parties, the Parties agree to amend the Contract as follows:
I. The Parties acknowledge and agree that the FY 16 Exhibit I in the current Contract shall
be replaced with the FY 16 Exhibit I as detailed below.
For and in consideration of the mutual assurances, promises, acknowledgments, warrants,
representations and agreements set forth in the Contract and this Amendment, and for other good
and valuable consideration, the receipt and adequacy of which is hereby acknowledged, the
undersigned hereby execute this Amendment on behalf of the CMH Authority and County and
by doing so legally obligate and bind the CMH Authority and County to the terms and
conditions of the Contract and this Amendment:

EXHIBIT I: Financial and Reporting Obligations
The funding made available under this contract is for the budget year October 1, 2015, through September
30, 2016. The authorized amounts are as contained in the Financial Status Report as detailed below.
The not-to-exceed amount of this agreement is $220,979. The source of funds is Federal Block Grants for
Substance Abuse and Mental Health Svc-Project of Regional and National Significance and the CFDA
number is 93.243. These funds are made available through the Michigan Department of Community
Health to the CMH Authority under federal grant award. This Interlocal agreement is designated as a sub-
recipient relationship and this is not a research and development project.
The County will be reimbursed monthly for actual expenditures incurred after the submission, review and
approval by the CMH Authority of each expenditure report. The expenditure reports must be submitted
on a monthly basis no later than twenty (22) days after the close of each calendar month.
A deviation allowance modifying an established budget category by $10,000.00 or 15 percent of the
funding for each project expenditure item, whichever is greater, is permissible. This deviation allowance
does not authorize new categories, subcontracts, equipment items, or positions not shown in Financial
Status Report.
The County shall submit to the CMH Authority quarterly reports on the progress on the key Strategies
and Activities outlined in Attachment A as applicable. The reports are due ten (10) days after the end of
each quarter.

Pursuant to Rule XII-C of this Board, the Finance Committee has reviewed the above referenced resolution
and finds:
1. The resolution authorizes amendment #2 of the Oakland County Community Mental Health Authority
(CMH Authority) interlocal agreement with the Oakland County Health Division (OCHD).
2. The amendment increases the total funding to $220,979 an increase of $57,349.
3. Grant funds will be used to train key community leaders from schools, law enforcement, the
community as well as youth and implement suicide prevention programming for youth.
4. The funding period will continue through September 30, 2016.
5. The amendment continues funding of one (1) Special Revenue (SR) Full Time Eligible Public Health
Educator II position (#11673).
6. The future level of service, including personnel, will be contingent upon the level of funding available.
7. The FY 2016 Special Revenue Budget is amended as follows:
