Chairperson, Ladies and Gentlemen:
WHEREAS Oakland University, in partnership with Oakland County Health Division and on behalf of the
Heathy Pontiac, We Can! Coalition, was awarded a three year grant by the Centers for Disease Control and
Prevention (CDC) titled Racial and Ethnic Approaches to Community Health (REACH) which was to end
September 30, 2017; and
WHEREAS the CDC is providing a one year extension to Oakland University of the REACH grant for the
period of October 1, 2017 through September 30, 2018; and
WHEREAS Oakland University wishes to amend its sub-contract under the REACH grant with Oakland
County Health Division to extend the project for an additional 12 months and to increase the budget amount to
cover two fiscal years; and
WHEREAS the Health Division will use the REACH grant funds to continue implementation of evidence and
practice-based strategy activities to reduce the devastating effects of obesity and chronic disease within the
City of Pontiac; and
WHEREAS with the 12 month extension, the new contract period is October 1, 2016 through September 30,
2018; and
WHEREAS the contract is not expected to be renewed after September 30, 2018; and
WHEREAS previous grant funding for this contract was in the amount of $129,306 for FY 2017; and
WHEREAS the amended budget is an increase of $78,369 for a two year funding period in an amount not to
exceed $207,675; and
WHEREAS the REACH grant will continue to fund one (1) special revenue (SR) full-time eligible (FTE) Public
Health Educator II position for this project by under-filling the currently vacant MCH Block Grant SR FTE
Public Health Nutritionist III position (#1060291-07360); and
WHEREAS this amendment has completed the Grant Review Process according to the Board of
Commissioners Grant Procedures and is recommended for approval.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners accepts
Amendment 1 to the REACH Grant Agreement from Oakland University in the amount of $207,675 that
includes a 12 month extension covering the contract period of October 1, 2016 through September 30, 2018.
BE IT FURTHER RESOLVED that the REACH grant will continue to fund one (1) SR FTE Public Health
Educator II position for this project by under-filling the currently vacant MCH Block Grant SR FTE Public
Health Nutritionist III position (#1060291-07360).
BE IT FURTHER RESOLVED that acceptance of this grant does not obligate the County to any future
commitment and continuation of the Special Revenue position in the grant is contingent upon continued future
levels of grant funding.
BE IT FURTHER RESOLVED that the Board Chairperson is authorized to execute the Grant Agreement and
approve changes and extensions not to exceed fifteen percent (15%).

Pursuant to Rule XII-C of this Board, the Finance Committee has reviewed the above referenced
resolution and finds:
1. The resolution authorizes Amendment 1 to the Centers for Disease Control and Prevention, FY
2017 Racial and Ethical Approaches to Community Health (REACH) sub-contract between
Oakland University and Oakland County Health Division (MR. #16281).
2. The amendment increases the total funding to $207,675, an increase of $78,369, and extends the
contract period for an additional 12 months; through September 30, 2018.
3. Funding will be used to continue one (1) Special Revenue (SR) Full-Time Eligible (FTE) Public
Health Educator II position.
4. The SR FTE Public Health Educator ll position will be under-filling one (1) SR FTE Public Health
Nutritionist III position (1060291-07360) which is currently vacant under the MCH Block Grant
Fund (#28563).

As approved by CDC, the Year 3 subcontract is being amended to include an increase in the
budget and an authorized redirection of funds. The total amount of the Year 3 budget has been

Budget Narrative/Justification
Year 3 Extension - October 1, 2016 - September 30, 2018
EXTENSION JUSTIFICATION: The Year 3 budget has been extended by 12 months (through
September 30, 2018) and approved by Centers for Disease Control and Prevention, The modified
subcontract distributes additional funding across all budget categories to Oakland County Health
Division needed to execute the project for an additional 12 month period. This includes funding a
FTE position dedicated to the project 100% through September 30, 2018 with additional funding
supporting previous personnel for work between Oct 2016 and AugOst 2017.
Attachment 1: Scope of Work

Oakland County Health Division is a key facilitator of the Healthy Pontiac, We Can!
coalition, and provides in-kind staff support to coordinate the coalition. They will continue
this in-kind support. Kristina Ottenwess, Health Education Supervisor will provide her
expertise in Health Communication to guide development of the Communications Plan and
will oversee communications-related activities (e.g., design of visual communication
strategies, ensuring consistency in messaging, etc.). Additionally, Shannon Brownlee will be
the staff support for maintaining and building the HPWC coalition including assessment,
strategic and sustainability planning, organization of coalition meetings, and engaging
community partners.
Given their expertise in health communications and experience in successfully managing
integrated community-wide media strategies, and to maintain consistency with current
HPWC communication strategies, Oakland County Health Division will be subcontracted to
manage development and implementation of communications plans for each proposed
activity.
Scope of Work: Oakland County Health Division (OCHD), under the leadership of
Oakland County Executive L. Brooks Patterson, organized the Healthy Pontiac, We Can!
(HPWC) coalition in 2011 to help the Pontiac community eat healthy, get active, and live
tobacco free. It is a diverse coalition which includes leaders in primary and preventive health

care, mental health, human service agencies, education, the religious community,
governmental agencies, and local residents.
HPWC, on behalf of Oakland County Health Division, has received funding and technical
support from a National REACH Coalition Community Transformation grant and multiple
Michigan Department of Health and Human Services grants to support these efforts, Given
their expertise in public health education, health communications and experience in
successfully managing integrated community-wide public health strategies, and to maintain
consistency with current HPWC communication strategies, Oakland County Health Division
will be subcontracted to manage development and implementation of communications and
health education related plans for each proposed activity.
For this project, Oakland County Health Division will lead communication activities
associated with all projects and for the Healthy Pontiac, We Can! coalition, including
developing, implementing, and evaluating written communications plans, design of all
communication activities, providing printing and mailing service, and reporting on
communications outcomes to the Program Coordinator. OCHD staff will also provide
development and implementation expertise to the Oakland University Program Coordinator,
Principal Investigator and other program staff to plan, and implement grant activities.
OCHD will continue employment and supervision of a full-time Public Health Educator who
works directly with community partners to implement the initiatives outlined in the REACH
grant. Kristina Ottenwess, Health Education Supervisor will provide expertise in Health
Communication and Health Education to guide development of the Communications Plan
and will oversee communications activities (e.g., design of visual communication strategies,
ensuring consistency in messaging, etc.). Additionally, Shannon Brownlee will be the staff
support for maintaining and building the HPWC coalition including, assessment, strategic
and sustainability planning, organization of coalition meetings, and engaging community
partners.
Method of Accountability: OCHD will be monitored by the PI and the Program
Coordinator, and the team will meet at least monthly to provide progress updates, and will
provide a monthly report of grant-related activities and communications.

Public Health Educator IT - $44,939 annual rate (1.0FTE) This position will be dedicated
to the project 100% and will be allocated as 1.0FTE for 13 months (2,240 hrs)
(September 1 2017-Sept 30 2018), totaling $48,684, with additional funding supporting a
full-time Public Health Educator IT between October 1,2016 through June 9,2017
utilizing 817 hrs totaling $17,419, and part-time work between June 9, 2017 and
September 1, 2017 estimated at $3,136. This position will be responsible for assisting the
Program Coordinator with operationalization of the REACH grant activities and
implementation of the Work Plan. With an excellent understanding of best practices in
community-based strategies to improve healthy eating and physical activity in diverse
populations, they will ensure research-based and innovative strategies are implemented
efficiently and successfully. They will work directly with partnering agencies to
determine appropriate logistics for program implementation, and to provide appropriate
training as necessary. They will serve as a point of contact for partnering agencies and
will be available to troubleshoot logistical issues. They will participate in relevant
community activities and will regularly visit sites to provide guidance and support to

community partners. This staff will lead communication activities associated with all
grant-related projects and will work with Kristina Ottenwess, Health Education
Supervisor, to develop and coordinate communications activities for the Healthy Pontiac,
We Can! coalition. This includes development and design of all communication
activities, providing printing and mailing services, and developing, implementing, and
evaluating written communications plans, hi collaboration with the Program Coordinator
(Allyson Forest) at Oakland University. They will also participate in evaluation activities
as time allows. This position will provide support to the Healthy Pontiac, We Can!
coalition and will attend all meetings.
Fringe Benefits - $43,448
Public Health Educator 11 FTE fringe benefits for FY18 (September 1, 2017-September
30, 2018) estimated at 73.59% of salary totaling $35,827 with additional funding
supporting fringe benefits for a full-time Public Health Educator II between October 1,
2016 through June 9, 2017 at 42.71% totaling $7,440, and part-time work between June
9, 2017 and September 1, 2017 at 5.78% estimated at $181.
Travel - $1,080
1n-state travel - Mileage is reimbursed at a rate of .54 cents per mile. Estimated at 20
miles/week over 50 weeks/year for 2 years (2,000 total miles).
Supplies - $50,400
Print Costs - $25,000: $10,000 General Project Printing Healthy eating and physical
activity communications campaigns/materials, lawn signs, brochures, flyers, doorknob
hangers. $15,000 Direct Mailers will be used promote Pop Up Produce Markets, and
community-wide physical activity initiatives. Printing is estimated at $2,500 for 25,700
pieces (every Pontiac household), X 6 mailings for a total of $15,000.
Postage — $12,000: $12,000 postage for direct mailers (see above). Estimated postage
for 25,700 pieces is $2,000 X 6 mailings = $12,000.

Materials and Supplies - $13,400:
Pop Up Produce Markets ($5,000) Supplies necessary to host one additional pop up
produce market site such as folding tables, canopy tents, signage, display shelves and
bins, dry erase boards/markets, etc. Healthy Bites Sustainability Supplies ($5,000)
Supplies needed to enhance sustainability of previous healthy bites interventions in
convenience stores, grocery stores, hospitals, schools, such as additional signage, display
stands to enhance product placement, healthy recipe cards, etc. Misc. project supplies
($3,400)— Project supplies to produce in-house marketing materials (Annually
$1,700/year- $875 poster/plotter/foam core materials, $345 lamination/mounting
supplies, $230 color ink, $250 miscallinous).
Other Categories - $43,508
Telephone Communications — $672. Cost of a desktop telephone for the Public Health
Educator, $364 annually.
Advertising-$28,900. $2,500: Promote Healthy Pontiac initiatives via online strategies
(social media advertisements, page boosts or another form of applicable advertising),
radio, television, and website updates to communicate REACH grant initiatives. $16,800
Billboard advertisements for Sprout and general healthy eating and physical activity
initiatives ($700/month/billboard, 24 billboards months). $9,600 for newspaper
advertising (12 total, $800 each full-page color advertisements for Sprout and general
healthy eating and physical activity messaging).
Computer-Information Technology Charges — $5,440. Cost of providing a computer,
computer maintenance and connection to the county network. Annual charge is $2,720
per computer x 1 computer.
Liability Insurance - $250 Cost of providing liability insurance. Approved by CDC in
Year 3 (Oct 1 2016-Sept 30 2017), but disallowed in Year 4 (Oct 1 2017-Sept 30 2018).

Indirect Costs - $8,246: Calculated at a rate of 11.91% of total salaries ($69,239) per
current cost allocation plan, between October 1, 2016-September 30, 2018. Per federal
OMB Circular No, A-87, Attachment E page 54 item D.1.b of the circular, our county is
not required to have an indirect cost agreement with our cognizant agency (HUD). The
agency only requires us to develop an indirect cost proposal in accordance with this
circular and to submit a copy if requested. Oakland County has requested an indirect cost
agreement with our cognizant agency, but they have refused signing the above.

REACH Update
Here is the message that we shared with all REACH awardees on the call that
took place today, February 27 at 3:00 ET.
This information updates what we presented to you on the REACH Program
Call on February 23, 2017.
As of today, the REACH 17 FOA has been pulled from mmEgrants.gov . and
there will not be a new REACH FOA posted.
CDC will fund a YEAR 4 for the current REACH awardees as a cost
extension. Guidance is forthcoming.
REACH awardees have already received funding for the current YEAR3 and
should continue to fully implement their workplans through September 29,
2017.
Important Note: your Project Officer does not have any additional
information other than what is shared here. Please hold your questions
until you have received the guidance.
