Chairperson, Ladies and Gentlemen:
WHEREAS Miscellaneous Resolution (MR) #15179 approved an interlocal agreement between the Oakland
County Community Mental Health Authority (now renamed Oakland Community Health Network, OCHN) and
Oakland County for the Health Division to address youth suicide prevention in Oakland County through training
of key community leaders from public health, schools, law enforcement, mental health and other community
sectors and implement suicide prevention programming for youth; and
WHEREAS OCHN has issued amendment #4 for the period of October 1, 2017 through September 30, 2018
in the amount of $120,629, a decrease of $16,833 from fiscal year (FY) 2017 funding of $137,462; and
WHEREAS this amendment is sufficient to continue funding one (1) Special Revenue (SR) Full Time Eligible
(FTE) Public Health Educator ll position #1060233-11673 that was created in MR #15179; and
WHEREAS this Amendment has completed the Grant Review Process in accordance with the Board of
Commissioners grant procedures and is recommended for approval.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners accepts the OCHN
amendment #4 of the Interlocal Agreement with Oakland County for suicide prevention services in the amount
of $120,629 for the period October 1, 2017 through September 30, 2018.
BE IT FURTHER RESOLVED that acceptance of this amendment does not obligate the County to any future
commitment and continuation of the special revenue position in the grant is contingent upon continued future
levels of grant funding.
BE IT FURTHER RESOLVED that the Board Chairperson is authorized to execute the Interlocal Agreement
Amendment and approve changes and extensions not to exceed fifteen percent (15%).

AMENDMENT #4
The Parties, Oakland County ("County") and The Oakland Community Health Network ("OCHN"),
agree and acknowledge that the purpose of this Amendment is to modify as provided herein and
otherwise continue the present contractual relationship between the Parties as described in their
current Suicide Prevention Services Contract, ("Contract").
In consideration of the mutual promises, representations, assurances, agreements, and provisions
in the Contract and this Amendment, the adequacy of which is hereby acknowledged by the
Parties, the Parties agree to amend the Contract as follows:
I Effective May 1,2017, Oakland County Community Mental Health Authority changed its
agency's name to Oakland Community Health Network (OCHN).
2. The new address for OCHN is 5505 Corporate Drive, Troy, MI 48098.
3. The Parties acknowledge and agree that the FY 17 Exhibit Tin the current Contract shall be
replaced with the FY 18 Exhibit I as detailed below.
4. The Parties acknowledge and agree that the FY 17 Exhibit II in the current Contract shall be
replaced with the FY 18 Exhibit IT as detailed below.
For and in consideration of the mutual assurances, promises, acknowledgments,
warrants, representations and agreements set forth in the Contract and this Amendment,
and for other good and valuable consideration, the receipt and adequacy of which
is hereby acknowledged, the undersigned hereby execute this Amendment on behalf
of the OCHN and County and by doing so legally obligate and bind the OCHN and
County to the terms and conditions of the Contract and this Amendment:

EXHIBIT I: Financial and Reporting Obligations
The funding made available under this contract is for the budget year October 1, 2017, through
September 30, 2018. The authorized amounts are as contained in the Financial Status Report as detailed
below.
The not-to-exceed amount of this agreement is $120,629. The source of funds is Federal Block Grants for
Substance Abuse and Mental Health Svc-Project of Regional and National Significance and the CFDA
number is 93.243. These funds are made available through the Michigan Department of Health and Human
Services to the OCHN under federal grant award. This Interlocal agreement is designated as a sub-recipient
relationship and this is not a research and development project.
The County will be reimbursed monthly for actual expenditures incurred after the submission, review and
approval by the OCHN of each expenditure report. The expenditure reports must be submitted on a monthly
basis no later than twenty (22) days after the close of each calendar month.
A deviation allowance modifying an established budget category by $10,000.00 or 15 percent of the funding
for each project expenditure item, whichever is greater, is permissible. This deviation allowance does not
authorize new categories, subcontracts, equipment items, or positions not shown in Financial Status Report.
The County shall submit to OCHN quarterly reports on the progress on the key Strategies and Activities
outlined in Attachment A as applicable. The reports are due ten (10) days after the end of each quarter.

FY18 TYSP Work Plan Goals and Objectives
Goal 1: Increase skills in the treatment and/or referrals of youth at risk for suicide to law
enforcement, healthcare, and schools, with an emphasis on continuity of care.
Objective 1.1: Coordinate and host research-based training for law enforcement in Oakland
County as a result of the completed needs assessment and building on existing training
infrastructure. Expected outcome, 50 law enforcement officers will complete training, using
training log/evaluations to measure.
• Partner with CIT and CIT-Y
• Enhance suicide prevention training at law enforcement academy

Objective 1.2: Implementation of suicide prevention strategies will continue with at least two
middle and two high schools in West Bloomfield, Holly, Farmington, and/or Farmington Hills
during the 2017/2018 school year. Expected outcome, 600 total students will complete SOS
training, using training log/evaluations to measure.
• Continue joint planning with school administration and local coalitions to determine
readiness and finalize implementation plan for youth suicide prevention activities in the
school.
• Continue conversation(s) about review and updating of mental health related policies at
both a school and district level that support youth mental health and suicide prevention.
• Alert Common Ground and private providers in area when suicide prevention activities
will occur, and determine that adequate capacity is present to provide follow-up care to
youth who may need evaluation by a mental health provider.
• Continue to engage counseling and social work staff in discussion regarding preparedness
to respond to students needing follow-up during or after suicide prevention activities.
Information obtained will be used to inform decisions about further trainings/education
needed.
• Inform middle school and high school staff of SOS (signs of suicide) curriculum.
• Inform parents of middle school and high school students in SOS (signs of suicide) and
collect completed evaluation tool.
• Train middle school and high school students in SOS (signs of suicide) and collect
completed evaluation tool. Obtain active consent for youth under age 18.
• Complete SafeTALK with middle school and high school staff on school crisis teams.
• Strategically market safeTALK T4T training to staff when offered at the community level.
• Identify staff, parents, and youth who may be interested in participating in further
community suicide prevention activities. Staff, parents, and youth will be identified
through participation in SOS and suicide prevention activities that occur for further
activities such as a youth task force, working with coalitions, and forum participation.

Objective 1.3: Implementation of CAMS (Collaborative Assessment and Management of
Suicidality) training for clinical behavioral health staff. Expected outcome, 50 clinical staff will
complete CAMS training, using training log/evaluations to measure.

complete training.
• Distribute trainings to identified clinicians.
• Complete evaluation materials required.

Objective 1.4: Implementation of suicide prevention strategies will continue with middle and high
schools in West Bloomfield middle and high schools during the 2017/2018 school year. Expected

grant team to determine readiness and finalize implementation plan for youth suicide
prevention activities in the school.
• Continue to engage West Bloomfield counseling and social work staff in discussion

prevention activities. Information obtained will be used to inform decisions about further
trainings/education needed.
• Begin discussion about the creation or strengthening of mental health related policies at
both a school and district level that support youth mental health and suicide prevention.
• Complete gatekeeper training(s) for West Bloomfield select middle school and high school
staff on school crisis teams
• Identify West Bloomfield staff, parents, and youth who may be interested in participating
in further community suicide prevention activities. Staff, parents, and youth will be
identified through participation in suicide prevention activities that occur at West
Bloomfield for further activities such as a youth task force, working with coalitions, and
forum participation.
• Point person: OCHD Staff
Objective 1.5: Implementation of suicide prevention strategies will begin with Oakland
Community College (OCC) during the 2017/2018 school year. Expected outcome, 60
staff/students will complete trainings (SOS, safeTALK or ASIST) in FY18. Measured by training
logs/evaluations.
• Engage in discussion with OCC school administration to identify needs related to youth
and young adult suicide prevention. Elicit recommendations for further assistance as
needed.
• Alert Common Ground and private providers in area when suicide prevention activities
will occur, and determine that adequate capacity is present to provide care to youth who
may need evaluation by a mental health provider.
• Provide resource list to OCC identifying public and private mental health or substance use
disorder practitioners who are accepting new patients.
• Engage OCC Counseling Center in discussion regarding readiness to respond to students
who might need follow-up during or after suicide prevention activities. Information
obtained will be used to inform decisions about further trainings/education needed.
• Train OCC staff/students in SOS, SAFE Talk, or ASIST and collect completed evaluation
tool. Obtain active consent for students under age 18.
• Identify OCC staff and students who may be interested in participating in further
community suicide prevention activities. Staff, parents, and students will be identified
through participation in gatekeeper trainings offered and suicide prevention activities that
occur at OCC for further activities such as the task force, working with coalitions, and
forum participation.

Goal 2: Conduct a community needs assessment in Oakland County, using Qualtrics, to gain
information on first responder's current strengths, areas for improvement, and goals for the
future.
Objective 2.1: Analyze data received through the needs assessments to begin 10/1/17. Data will
be available to use to inform suicide prevention activities through FY18.
• Hand score qualitative data.
• Score quantitative data using the Qua'tries program.
• Review and summarize findings from needs assessment.
• Develop final report with recommendations
• Revise work plan as needed to incorporate findings

Goal 3: Develop and implement policies and procedures surrounding suicide prevention in
Oakland County.
Objective 3.1: Oakland County will convene and facilitate a Zero Suicide Network to enhance
and expand Zero Suicide policies and procedures.
• Collaborate with SPRC staff for technical assistance.

Objective 3.2: Implement Zero Suicide toolkit/activities at organizations attended the Zero
Suicide workshop.
• Review resources for Zero Suicide online and will bring those resources back to partner
agencies to begin implementing these activities in FYI 8.

Objective 3.3: Draft and promote templates of suicide prevention policies / procedures / strategies
to share with community agencies and organizations.
• Taskforce members will assist in drafting, approving, and promoting suicide prevention
related policies, procedures, strategies to community agencies.
• Provide technical assistance to organizations looking to enhance and implement suicide
prevention strategies in their agencies.
Goal 4: Increase knowledge and skills for Means Restrictions through education, training,
and awareness.
Objective 4.1: Implement best practices in means restriction education and training. Expected
outcome, 100 of community members will complete training, using training log/evaluations to
measure.
• Implement Counseling on Access to Lethal Means (CALM) training by distributing to
behavioral health network and professionals through emails, website, social media, etc.
• Evaluate and track registrations

Objective 4.2: Address means restriction through education and distribution of resources through
community events and implementation of a multi-component community wide campaign
encouraging residents to restrict access to lethal means.
• Partner with SAC and DEA to distribute lock bags in collaboration with DEA take back

listserve, etc.
Objective 4.3: Create a research-based, online training for community residents and other non-
clinical professionals to educate about restricting access to lethal means.

Goal 5: Address stigma through education and promotion of resources through community
events and implementation of a multi-component community wide campaign encouraging

Objective 5.2: Host two community events:
• Youth event in the fall of 2017. Focus is on promoting the youth voice and giving the
opportunity for them to identify needs related to suicide prevention. Expected outcome,
attendance of 150 measured by log/evaluation.
o Partner with Alliance of Coalitions for Healthy Communities (ACE-IC) to co-
sponsor and obtain optimal youth participation. This will occur through joint
meetings between the two entities. A contract will be created between the grant
(OCHN) and ACHC. Grant will financially support, promote, and attend the
event.
o Parents complete an online active consent form facilitated by ACHC.
o Point Person: OCHN
• Community forum in the spring of 2018. Focusing on suicide prevention and
supporting care coordination from both the professional and family perspective in
partnership with the Youth Suicide Prevention Task Force in Oakland County.
Expected outcome, attendance of 80 community members. Measured by attendance
log/evaluation.
o Gather information from forum participants through group discussion and
verbal, written and/or online feedback on youth suicide prevention in Oakland
County.
o Review/discuss information received at forums with the task force and use it to
inform youth suicide prevention work in Oakland County.

Objective 5.3: Promote participation of school staff and community members in the Youth
Mental Health First Aid Trainings that will be occurring free of charge through the Oakland
County ISD, sponsored by Project AWARE. This will be occurring ongoing throughout this fiscal
year. Expected outcome, a minimum of 125 community members who engage with youth on daily
basis will become trained in YMHFA, to be measured by training logs.
• Obtain schedule of YMHFA trainings Project AWARE is sponsoring and assist in
promoting/advertising as needed through social media, email, etc.

Objective 5.4: Promote and distribute Common Ground's suicide prevention hotline and website
(http://www.suicideprevention.commongroundhelps.org ). Expected outcome, to see a 10%
increase in use of the suicide prevention hotline in FYI 8, measured by data provided by Common
Ground- using the following means:
• Include these resources in the youth suicide prevention toolkits, prescription drug toolkits,
and other relevant materials. This will be added to the toolkits before the next reprint.
• Press release
• Social media messaging
• Enhance and expand the teen chat line to 24 hours a day, 7 days a week. Work with
Common Ground on a plan for sustainability to maintain the teen chat line.

Objective 5.5: Promote and distribute OK2SAY website, call and text tipline
(http://vvww.michigan.gov/ok2say/) Expected outcome to see a 10% increase in use of the tip line
in FY18, measured by data provided by OK2SAY/Michigan Attorney General-using the following
means:
• Include these resources in the youth suicide prevention toolkits, prescription drug toolkits,
and other relevant materials. This will be added to the toolkits before the next reprint.
• Press release
• Social media messaging
• Promotion during youth event in FY18 in partnership with ACHC and Project AWARE.
• Point Person: OCHN / OCHD staff
Goal 6: To collaborate with other systems and grants within the community on complimentary
projects and work towards improved continuity of care.
Objective 6.1: Continue to build/strengthen relationships with community organizations and
collaborations in Oakland County.
• Continue to collaborate with the Youth Suicide Prevention Task Force in Oakland County
to support and participate in activities listed below. Expected outcome will be
representation of the health division and/or OCHN at all task force meetings, measured by
meeting minutes.
o Complete stakeholder mapping to broaden the sectors represented on the task force
and strengthen collaboration among current partners. Information for mapping will
come from current task force members' input/discussion.
o Youth Suicide Prevention Community Forum, assist with preparation, set up, event,
and evaluation.
o Review and update school toolkit with input from at least 2 districts within the
county (1 rural, 1 urban). Translation of parent toolkits to Spanish and Arabic as
needed.
o Ensuring toolkits are accessible to schools in the community, updating and
distributing toolkits as needed. The task force will draw on members who work for
districts and the ISD to determine what schools need toolkits, getting the word out
to schools that they are available, etc.
o Point person: OCHD Staff
• Continue collaboration with Project Aware regarding each grant's goals and objectives,
training opportunities, etc. This collaboration will be ongoing throughout the 5 years of
the grant. To be measured by a minimum of 2 collaborative meetings between the grants
to occur in FY18 measured by meeting minutes/attendance.
o Participate as a member of the Project AWARE community management team.
o Project AWARE will have representation on the task force.
o Communicate with one another regarding upcoming activities to cross promote
each other's efforts. This will include face to face meetings, phone contact, and
email.
o Point Person: OCHN staff
• Continue collaboration with Jewish Family Services regarding each grant's goals and
objectives, training opportunities, etc. This collaboration will be ongoing throughout the 5
years of the grant. To be measured by a minimum of 2 collaborative meetings between the
grants to occur in FY18 measured by meeting minutes/attendance.
o Jewish Family Services will have representation on the task force.
o Communicate with one another regarding upcoming activities to cross promote
each other's efforts. This will include face to face meetings, phone contact, and
email.

• Collaborate with Oakland County Community Mental Health Juvenile Justice Liaison to
share information about grant initiatives and establish relationship. To be measured by a
minimum of 1 collaborative meeting to occur in FY18 measured by meeting
minutes/attendance.

Objective 6.2: Continue building relationships with healthcare providers in Oakland County to
increase opportunities for training and collaboration around suicide prevention. Expected
outcome, TYSP staff will attend community meetings (see below activities) with healthcare
providers, minimally quarterly to provide information on grant activities/opportunities for
healthcare providers. Measured by meeting minutes/agenda/ attendance.
• Continued participation on the task force by TYSP grantees.
• Attending prescription drug abuse partnership meetings at the Health Division by TYSP
grantees and providing updates and information on the grant.
• Attending and providing information about the grant at the clinical best practices group
meeting.
Objective 6.3: Attend and participate in professional development opportunities related to grant
goals.
• A community team from Oakland County will attend the suicide prevention TA meeting
together in Roscommon, MI. To be completed by 6/1/18. To be measured by attendance
at the TA meeting.
• Oakland Community Health Network and Oakland County Health Division will send one
staff to the Grantee GLS meeting in Washington DC in spring of FY18. Expected outcome
to increase knowledge and skill in the area of strategies, trainings, and implementation
related to youth suicide prevention of TYSP grantees that can be brought back to Oakland

Pursuant to Rule XII-C of this Board, the Finance Committee has reviewed the above referenced resolution
and finds:
1. The resolution authorizes amendment #4 of the Oakland Community Health Network (formally known
as the Oakland County Community Mental Health Authority) interlocal agreement with the Oakland
County Health Division in the amount of $120,629.
2. Grant funds will be used to train key community leaders from schools, law enforcement, the
community and implement suicide prevention programming for youth.
3. The funding period is October 1, 2017 through September 30, 2018 and is anticipated to continue
through FY 2019.
