Chairperson, Ladies and Gentlemen:
WHEREAS Miscellaneous Resolution (MR) #15179 approved an interlocal agreement between the
Oakland County Community Mental Health Authority (now renamed Oakland Community Health Network,
OCHN) and Oakland County for the Health Division to address youth suicide prevention in Oakland County
through training of key community leaders from public health, schools, law enforcement, mental health and
other community sectors and implement suicide prevention programming for youth; and
WHEREAS this amendment concludes the 5-year funding (10/1/14 through 9/30/19) that OCHN received
from the Michigan Department of Health and Human Services (MDHHS) for the youth suicide prevention
program; and
WHEREAS OCHN has issued amendment #6 for the period of October 1, 2018 through September 30,
2019 in the amount of $129,080, an increase of $8,451 from fiscal year (FY) 2018 funding of $120,629; and
WHEREAS this amendment is sufficient to continue funding one (1) Special Revenue (SR) Full Time
Eligible (FTE) Public Health Educator ll position #1060233-11673 that was created in MR #15179; and
WHEREAS this Amendment has completed the Grant Review Process in accordance with the Board of
Commissioners grant procedures and is recommended for approval.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners accepts the
OCHN amendment #6 of the Interlocal Agreement with Oakland County for suicide prevention services in
the amount of $129,080 for the period October 1, 2018 through September 30, 2019.
BE IT FURTHER RESOLVED that acceptance of this amendment does not obligate the County to any
future commitment and continuation of the special revenue position in the grant is contingent upon
continued future levels of grant funding.
BE IT FURTHER RESOLVED that the Board Chairperson is authorized to execute the Interlocal Agreement
Amendment and approve changes and extensions not to exceed fifteen percent (15%).

The Parties, Oakland County ("County") and The Oakland Community Health Network ("OCHN"),
agree and acknowledge that the purpose of this Amendment is to modify as provided herein and
otherwise continue the present contractual relationship between the Parties as described in their
current Suicide Prevention Services Contract, ("Contract").
In consideration of the mutual promises, representations, assurances, agreements, and provisions
in the Contract and this Amendment, the adequacy of which is hereby acknowledged by the
Parties, the Parties agree to amend the Contract as follows:
1. The Parties acknowledge and agree that the FY 18 Exhibit Tin the current Contract shall be
replaced with the FY 19 Exhibit I as detailed below.
2. The Parties acknowledge and agree that the FY18 Exhibit II in the current Contract shall be
replaced with the FY19 Exhibit II as detailed below.
For and in consideration of the mutual assurances, promises, acknowledgments,
warrants, representations and agreements set forth in the Contract and this Amendment,
and for other good and valuable consideration, the receipt and adequacy of which is
hereby acknowledged, the undersigned hereby execute this Amendment on behalf of
the OCHN and County and by doing so legally obligate and bind the OCHN and
County to the terms and conditions of the Contract and this Amendment:

EXHIBIT I: Financial and Reporting Obligations
The funding made available under this contract is for the budget year October 1, 2018, through
September 30, 2019. The authorized amounts are as contained in the Financial Status Report as detailed
below.
The not-to-exceed amount of this agreement is $129,080. The source of funds is Federal Block Grants for
Substance Abuse and Mental Health Svc-Project of Regional and National Significance and the CFDA
number is 93.243. These funds are made available through the Michigan Department of Health and Human
Services to the OCHN under federal grant award. This Interlocat agreement is designated as a sub-recipient
relationship and this is not a research and development project.
The County will be reimbursed monthly for actual expenditures incurred after the submission, review and
approval by the OCHN of each expenditure report. The expenditure reports must be submitted on a monthly
basis no later than twenty (22) days after the close of each calendar month.
A deviation allowance modifying an established budget category by $10,000.00 or 15 percent of the funding
for each project expenditure item, whichever is greater, is permissible. This deviation allowance does not
authorize new categories, subcontracts, equipment items, or positions not shown in Financial Status Report.
The County shall submit to OCHN quarterly reports on the progress on the key Strategies and Activities
outlined in Exhibit II as applicable. The reports are due ten (10) days after the end of each quarter.

Exhibit II: Scope of Services
FY19 TYSP Work Plan Goals and Objectives
Goal 1: Increase skills in the treatment and/or referrals of youth at risk for suicide to law
enforcement, healthcare, and schools, with an emphasis on continuity of care.
Objective 1.1: Coordinate and host research-based training for law enforcement in Oakland
County as a result of the completed needs assessment and building on existing training
infrastructure. Expected outcome, 30 law enforcement officers will complete training, using
training log/evaluations to measure.
• Enhance suicide prevention training with law enforcement through OCHN's CIT training
• Provide suicide prevention training at CIT training to approximately 30 law enforcement
officers
• Set meeting with Oakland County Sheriffs office to discuss enhancement of suicide
prevention training during academy training.
• Point Person: OCHD Staff
Objective 1.2 Provide technical assistance on suicide prevention with first responder
organizations, based on needs assessment results. Expected outcome three collaborative meetings
with the top responders of the First Responders Needs Assessment documented by meeting
minutes.
• Set meeting with director of Oakland County Medical Control Authority to discuss needs
assessment results and next steps.
• Attend Oakland County First Responder meeting to present results, best practices for
policies and procedures, and to discuss available trainings.
• Set meeting with top 3 agencies (Rochester Hills Fire Department, Star EMS, and
Waterford Township Fire Department) who responded to the survey, to discuss results and
next steps.
• Point Person: OCHD Staff
Objective 1.3 Implementation of intermediate suicide prevention strategies will continue with
middle, high school and college level schools during the 2018/2019 school year. Expected
outcome, 80 staff will complete safeTALK training, using training log/evaluations to measure.
• Continue planning with two school districts to determine readiness and finalize
implementation plan for youth suicide prevention activities in the school.

Objective 1.4 Implementation of peer gatekeeper suicide prevention strategies will continue with
middle and high school students during the 2018/2019 school year. Expected outcome, 400
students will complete SOS training in FY19. Measured by training logs/evaluations.
• Train middle school or high school students in SOS and collect completed evaluation tool.
• Obtain active consent for students under age 18.
• Share resources with parents prior to and after SOS training takes place in their child's
school.

Goal 2: Develop and implement Zero Suicide in Oakland County.
Objective 2.1: Oakland County will convene and facilitate a quarterly Southeast Michigan Zero
Suicide Network activity to enhance and expand Zero Suicide policies and procedures. Measured
by quarterly documentation of activities.
• Collaborate with SPRC staff for technical assistance.
• Draft and promote powerpoints, policies, procedures and provide assistance to agencies
creating their own

Objective 2.2: Implement Zero Suicide toolkit/activities at OCHD and OCHN (and affiliated
agencies). Measured by quarterly documentation of activities.
• Continue implementing Zero Suicide Workplan through FY19.
• Collaborate with SPRC staff for technical assistance.

Goal 3: Increase community education/awareness regarding mental health and suicide
prevention
Objective 3.1: Promote online lethal means training for the general public. Expected outcome, 50
participants will complete the training.
• Promote online training via social media, govdelivery, speaker's team, etc.
• Track the number of participants via coursemill
• Point person: OCHD staff
Objective 3.2: Implement safeTALK trainings in the community. Expected outcome, 80
community members will complete safeTALK training, using training log/evaluations to measure.
• Partner with community agencies to host safeTALK throughout the county
• Point person: OCHD staff
Objective 3.3: Implement two ASIST trainings in the community. Expected outcome, two
trainings using training logs and evaluations to measure.
• Partner with community agencies to host ASIST throughout the county.
• Point person: OCEID staff
Objective 3.4: Four local trainers will be trained in ASIST through a Training for Trainers.
Expected outcome four local trainers in the ASIST model.
• Two staff from OCHD and two staff from OCHN will complete 5 day training for trainers
of the ASIST model.
• Local trainers will complete a training plan as part of the sustainability plan.

Goal 4: Address stigma through education and promotion of resources through community
events and implementation of a multi-component community wide campaign encouraging
residents to seek help and talk to someone they trust.
Objective 4.1: Educate community through various print and media channels. Expected outcome,
reach 20,000 people.
• Print suicide prevention materials, disseminate through various events, community
organization trainings etc.
• Create advertising materials i.e., billboard design, bus ads, social media posts etc.
• Press release on the project from OCHD

Objective 4.2: Support a youth event.
• Youth event in the fall of 2018. Focus is on promoting the youth voice and giving the
opportunity for them to identify needs related to suicide prevention. Expected outcome,
attendance of 150 measured by log/evaluation.
• Partner with Alliance of Coalitions for Healthy Communities (ACHC) to co-sponsor and
obtain optimal youth participation. A contract will be created between the grant (OCHN)
and ACHC. Grant will financially support, promote, and attend the event.
• Parents complete an online active consent form facilitated by ACHC.
• Point Person: OCHN
Objective 4.3: Host a Community forum in the spring of 2019. Focusing on suicide prevention
and supporting care coordination from both the professional and family perspective in partnership
with the Youth Suicide Prevention Task Force in Oakland County. Expected outcome, attendance
of 80 community members. Measured by attendance log/evaluation.
• Gather information from forum participants through group discussion and verbal, written
and/or online feedback on youth suicide prevention in Oakland County.
• Review/discuss information received at forums with the task force and use it to inform
youth suicide prevention work in Oakland County.
• Point Person: OCHD staff
Objective 4.4: Initiate the creation of a MindKare kiosk program at Oakland Community College
(OCC) in partnership with Easterseals Michigan to screen for mental health and link to quality
treatment options. Expected Outcome, 150 individuals take screening for the fiscal year.
• Create a MindKare kiosk program at OCC
• Support advertising of kiosk.
• Gather data monthly from kiosk.
• Point Person: OCHN
Goal 5: To collaborate with other systems and grants within the community on complimentary
projects and work towards improved continuity of care.
Objective 5.1: Continue to build/strengthen relationships with community organizations and
collaborations in Oakland County, Expected outcome will be representation of the health division
and/or OCHN at all task force meetings, measured by meeting minutes.
• Continue to collaborate with the Youth Suicide Prevention Task Force in Oakland County.
• Implement Youth Suicide Prevention Task Force Strategic Plan with task force members
• Point person: OCHD and OCHN Staff
Objective 5.2 Continue collaboration with Project Aware regarding each grant's goals and
objectives, training opportunities, etc. This collaboration will be ongoing throughout the 5 years
of the grant. To be measured by a minimum of 2 collaborative meetings between the grants to
occur in FYI 9 measured by meeting minutes/attendance.
• Participate as a member of the Project AWARE community management team.
• Project AWARE will have representation on the task force.
• Communicate with one another regarding upcoming activities to cross promote each other's
efforts. This will include face to face meetings, phone contact, and email.
• Point Person: OCHN and OCHD staff
Objective 5.3: Attend and participate in professional development opportunities related to grant
goals.
• A community team from Oakland County will attend the suicide prevention TA meeting
together in Roscommon, MI. To be completed by 6/1/18. To be measured by attendance
at the TA meeting.
• Oakland Community Health Network and Oakland County Health Division will send staff
to Kevin's Song Conference. Expected outcome to increase knowledge and skill in the area
of strategies, trainings, and implementation related to youth suicide prevention of TYSP
grantees that can be brought back to Oakland County. Measure will be attendance.

Goal 6: Implement DBT (Dialectical Behavioral Therapy), an evidence based practice therapy
approach, in a clinical setting for adolescents and their caregivers to reduce suicide and self-
harm behaviors throughout the OCHN provider network
Objective 6.1 Coordinate technical assistance for the first 6 months of implementation for 3 new
DBT teams. Measured by monthly coaching calls for the 6 months of implementation.
• Create a contract with the Michigan DBT Institute of Michigan to provide technical
assistance and coaching.
• Organize technical assistance coaching calls with the Michigan DBT Institute of
Michigan and the provider agencies
• Support teams in attending technical assistance coaching calls.

Objective 6.2 Coordinate a site visit for each new DBT team. To be measured by a site visit for
each new DBT team.

Goal 7: Enhance collection of suicide-related data.
Objective 7.1 OCHN will collect data related to suicide attempts within the OCHN network and
review results. Expected outcome, quarterly data reports.

code (R1!)
• OCHN will report to Zero Suicide Work Group quarterly on data
• OCHN will put all data together at the end of the fiscal year and identify any trends

Objective 7.2 Increase collaboration with medical examiner's office to gather accurate suicide
death data. To be measured by a minimum of 2 collaborative meetings between the grants to
occur in FY19 measured by meeting minutes/attendance.

• Representative from ME's office will participate on the task force.

Objective 7.3 Implement the Oakland County Suicide Mortality Review Team by using the
Psychological Biography procedure. To be measured by 2 collaborative meetings to occur in
FY1 9.

• Set meetings with key partners to be involved in suicide mortality review team

Goal 8: Establish strategies to sustain grant initiatives beyond grant funding period.
Objective 8.1 Create a sustainability plan document. To be measured by completed
sustainability plan document approved by both OCHD and OCHN.
• Set meetings with OCHD and OCHN staff as well as other key players in suicide
prevention in Oakland County.
• Create a suicide prevention trainer network.

Pursuant to Rule XII-C of this Board, the Finance Committee has reviewed the above referenced resolution
and finds:
1. The resolution authorizes amendment #6 of the Oakland Community Health Network (formally
known as the Oakland County Community Mental Health Authority) interlocal agreement with the
Oakland County Health Division in the amount of $129,080; an increase of $8,451 from the FY
2018 award of $120,629.
2. Grant funds will be used to train key community leaders from schools, law enforcement, the
community and youth as well as implement suicide prevention programming for youth.
3. The funding period is October 1,2018 through September 30, 2019.
4. This amendment continues funding one (1) Special Revenue (SR) Full Time Eligible Public Health
Educator II position (#1060233-11673).
5. The future level of service, including personnel, will be contingent upon the level of funding
available.
