Taub, Chairperson, General Government Committee RE: Department of Human Services/Medical Care Facility — Acceptance of Additional Funding for the Nursing Home Quality Incentive Grant — Greater than 15% TO: Oakland County Board of Commissioners Chairperson, Ladies and Gentlemen: WHEREAS, the Michigan Department of Consumer and Industry Services has made additional funding available in the amount of $4,440.00 through the Nursing Home Quality Incentive Grant program, for the period of October 1, 1999 through September 30, 2000; and WHEREAS, the conditions, policies and procedures outlined in correspondence dated February 28, 2000 as well as the understanding communicated in the letter of April 20, 2000 for the original amount of $24,960.00 apply to this award as well; and WHEREAS, it is expected all monies will be spent on the previously approved innovative projects. NOW THEREFORE BE IT RESOLVED THAT the Oakland County Board of Commissioners does hereby approve the acceptance of the additional $4,440.00 in the Nursing Home Quality Incentive Grant program funding provided through the Michigan Department of Consumer and Industry Services. A check for $24,960.00 will be issued shortly to your facility, based on $208.00 for each licensed bed. Note that this represents a change in the previously set amount of $188.00 per bed. The department will expect that the additional amount included in your grant award be justified by your facility within the confines of your approved project. The approved grant is in part for your proposal to obtain services related to the Eden Alternative ($20,125.11). The Office of Services to the Aging within the Michigan Department of Community Health will be sending out a schedule, registration materials and information about how to obtain the services for which you were approved. Please do not go forward with your Eden-related project until you hear from the Office of Services to the Aging. Furthermore, the remaining amount of $4,834.89 is approved for your innovative projects: 1. Improve resident nutritional status by enhancing dining room experience 2. Staff communication system-Message board Please be aware that the following points must be adhered to throughout the grant period: • For any physical plant improvements, construction permits may be required. Check with your local building inspection authority to determine permit requirements prior to any construction. • Prudent business practices are expected for all transactions involving grant monies. Be sure to obtain as many cost estimates as necessary to ensure that you are not paying higher than market price for any goods or services. • All expenses must be fully accounted for at the end of the state's fiscal year, September 30, 2000. Please complete and return the enclosed Financial Report no later than October 16, 2000. All expenses related to your innovative project must be fully accounted for in the Financial Report. ,e2 , • iter to Ms. Shirla F. Kugler Be sure to keep all receipts related to your innovative project and supply a photocopy of the receipts with the completed Financial Report. Please note that the Financial Report defines an "encumbrance" as a "contract for goods not received as of 9/30/00 representing a legal obligation of the grantee to pay a specified sum upon receipt of the goods." An encumbrance only pertains to goods, not services, for purposes of this grant program. If you have any questions about completing the Financial Report, please contact Ms. Toni Randolph in the department's Finance and Administrative Services Bureau by telephone at (517) 335-1969. Any questions related to Medicaid payments should be directed to the Medical Services Administration within the Michigan Department of Community Health. A separate Grant Program Evaluation form will be forwarded to you prior to the end of the fiscal year. This evaluation is completely separate from the Financial Report discussed above. For purposes of this evaluation, please be sure to do the following: Keep track of the "before" and "after" results of your project: Be able to provide information on any measurable changes at the end of the fiscal year by having specific baseline information related to the impact of your project, such as resident health status; resident activity; resident participation; staff turnover; staff training and development or other anticipated impacts now and as of September 30, 2000. - Review the expected impact of your project and be prepared to report on whether it was achieved. Be prepared to support your statements with as much documentation as possible, including resident or family council minutes; photographs of purchased equipment or physical improvements; statistics on increased staffing ratios or retention, etc. - Be prepared to repeat your resident satisfaction assessment after the end of the fiscal year. On February 28, 2000 the Medical Care Facility received a grant award in the amount of $24,960 to implement a quality program. This award was processed through the County's Grant Application/Acceptance process, resulting in the approval of MR #00092 on May 4, 2000. 2. On May 23, 2000 the Medical Care Facility was notified by the Michigan Department of Consumer & Industry Services that an additional amount of $4,440 was to be awarded under the Nursing Home Quality Incentive Grant Program. 3. The conditions, policies and procedures outlined in the original grant application/acceptance apply to this additional funding'. 4. The grant period runs from October 1, 1999 through September 30,2000. 5. A County match is not required, and no new staff is requested. 6.
