Chairperson, Ladies and Gentlemen:
The General Government Committee, having reviewed Miscellaneous Resolution
#03149 on July 7, 2003, reports with a recommendation that the resolution be adopted.
Chairperson, on behalf of the General Government Committee, I move
acceptance of the foregoing report.

IN RE: WEST NILE VIRUS FUND REIMBURSEMENT FOR PROJECT IN THE CITY OF
HAZEL PARK - FISCAL YEAR 2003 ALLOCATION
TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Chairperson, Ladies and Gentlemen:
WHEREAS the Oakland County Board of Commissioners established a
West Nile Virus Fund Disbursement Policy per Miscellaneous Resolution
403060 in an effort to assist cities, villages and townships (CVTs)
with mosquito control measures to prevent the spread of West Nile Virus
and to help protect the health, safety and welfare of the people of
Oakland County; and
WHEREAS pursuant to the Disbursement Policy, the City of Hazel
Park has submitted a resolution of authorization to the County for
reimbursement of expenses incurred in connection with an eligible
mosquito control project; and
WHEREAS the FY 2003 authorized amount of funding for the City of
- Hazel Park is $6,340.46 from the West Nile Virus Fund as repayment to
the City of Hazel Park for expenses incurred in connection with
mosquito control projects.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of
Commissioners approves the project submitted by the City of Hazel Park
as eligible for reimbursement from the West Nile Virus Fund.
BE IT FURTHER RESOLVED that the Board authorizes the FY 2003
appropriation in the amount of $6,340.46 from the West Nile Virus Fund
(Account 490-210000-23030-9098) to repay the City of Hazel Park for
expenses incurred in connection with the mosquito control project, once
proper invoices are presented.

Dear Mr. Soave:
I am in receipt of a West Nile Virus fund reimbursement request from the City of
Hazel Park dated May 28, 2003. After having reviewed their documentation, I
find them in compliance with Oakland County Board of Commissioners
Miscellaneous Resolution 03060 and in compliance with at least one or more of
the four major categories for reimbursement. This letter will, therefore, certify
that all expenses submitted for reimbursement are for qualifying mosquito control

Re: West Nile Virus Fund Reimbursement Request
Pursuant to Oakland Board of Commissioner Miscellaneous Resolution (MR) 03060,
the City of Hazel Park herby requests reimbursement under Oakland County's West
Nile Virus Fund Program for expenses incurred in connection with an eligible
mosquito control activity by our City.
Attached in support of this request are the following:
1. ATTACHMENT A: PROJECT DESCRIPTION
Public Education Code Enforcement clean all catch basin's city wide &
larvicide.
2. ATTACHMENT B: GOVERNING BODY RESOLUTION
Attached is a certified copy of the local governing body's resolution
authorizing the reimbursement request.
3. ATTACHMENT C: EXPENSE INVOICES
Attach are copies of bills/invoices reflecting CVT expenses for which
reimbursement is sought. (If pre-project authorization is used, invoices
will be sent after work completion).
The City understands that the review and processing of this reimbursement request
will be governed by the provisions of MR 03060, and certifies that all expenses
submitted for reimbursement were incurred in connection with a qualifying mosquito
control project.

CITY OF HAZEL PARK
RESOLUTION
WHEREAS, the Mayor and Council of the City of Hazel Park support the efforts
of Oakland County in its elimination of the West Nile Virus; and
WHEREAS, of October 11, 2002. the Center for Disease Control reported 577
positive cases of West Nile Virus in humans in the State of Michigan, with 51
associated deaths. Of these, Oakland County had 30 reported human deaths —
the highest in the State; and
WHEREAS, the concentrated outbreak of West Nile Virus cases in Oakland
County merit instituting proactive public health measures, such as personal
protection along with habitat eradication, larviciding and focused mosquito
spraying to reduce the population of infected mosquitoes in the environment; and
• WHEREAS, the Mayor and Council of the City of Hazel Park wish to continue its
support of such programs for the protection of its residents;
NOW THEREFORE BE IT RESOLVED, the Mayor and City Council of Hazel
Park on behalf of not only it's own residents but those of neighboring cities,
respectfully request mutual assistance from the established fund through it's
Board of Commissioners, Department of Management and Budget, Fiscal
Services Division as to help protect the health, safety and welfare of the people
of Oakland County and residents cf the City of Hazel Park.

Pursuant to Rule XII-C of this Board, the Finance Committee has
reviewed the above referenced resolution and finds:
1. Funding of $6,340.46 is available in the West Nile Virus
Fund for FY 2003 to reimburse the City of Hazel Park for
this project; no additional appropriation is required.
2. The Committee recommends adoption of this resolution.
