Miller, MA Manager/Health Officer Date: May 30, 2003 Re: Lake Angelus West Nile Virus Fund Reimbursement Request I am in receipt of a request from the City of Lake Angelus dated April 9, 2003. In that letter they are requesting reimbursement in the amount of $327.71, which is the amount designated for Lake Angelus. There are a couple of problems with their request. First, they do not have a project description and second, they do not have an actual expense invoice. Instead, what they have attached is a quote from Advanced Pest Management which indicates that there will be some site inspections done of their catch basins, as well as some larvacide application done. However, I do not have any evidence that this company was cortracted with. Due to the fact that this is such a small amount, I did not know if you want.xl to hold them to the supporting documentation requirements. However, it is my firm beli:f that if we Jet one go someone else will come in with missing data that is requesting more monies and this could become a problem. If you would like to discuss this, please contact me at 8-1410 and I would be happy to work through a resolution with you.
