adopted.

IN RE: WEST NILE VIRUS FUND REIMBURSEMENT FOR PROJECT IN THE CITY OF
MADISON HEIGHTS - FISCAL YEAR 2004 ALLOCATION
TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Chairperson, Ladies and Gentlemen:
WHEREAS the Oakland County Board of Commissioners via
Miscellaneous Resolution 404120 approved the extension of the West Nile
Virus Fund for FY 2004; and
WHEREAS the Oakland County Board of Commissioners established a
West Nile Virus Fund Disbursement Policy per Misrpllaneous Resolution
403060 in an effort to assist cities, villages and townships (CVTs)
with mosquito control measures to prevent the spread of West Nile Virus
and to help protect the health, safety and welfare of the people of
Oakland County; and
WHEREAS pursuant to the Disbursement Policy, the City of Madison
Heights has submitted a resolution of authorization to the County for
reimbursement of expenses incurred in connection with an eligible
mosquito control project; and
WHEREAS the FY 2004 authorized amount of funding for the City of
Madison Heights is $10,744.01 from the West Nile Virus Fund as
repayment to the City of Madison Heights for expenses incurred in
connection with mosquito control projects.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of
Commissioners approves the project submitted by the City of Madison
Heights as eligible for reimbursement from the West Nile Virus Fund.
BE IT FURTHER RESOLVED that the Board authorizes the FY 2004
appropriation in the amount of $10,744.01 from the West Nile Virus Fund
(Account 490-210000-23030-9098) to repay the City of Madison Heights
for expenses incurred in connection with the mosquito control project,
once proper invoices are presented.
Chairperson, I move the adoption of the foregoing resolution.

Dear Mr. Soave:
Pursuant to Oakland County Board of Commissioner Miscellaneous Resolution (MR) 04120, the
City of Madison Heights hereby requests reimbursement under Oakland County's West Nile
Virus Fund Program for expenses incurred in connection with an eligible mosquito control
activity by our City.
Attached in support of this request are the following:

2. Attachment B: City Council Resolution authorizing reimbursement request.
3. Attachment C: Expense Invoices
The City of Madison Heights understands that the review and processing of this reimbursement
request will be governed by the provisions of MR 04120, and certifies that all expenses
submitted for reimbursement were incurred in connection with a qualifying mosquito control
project.

West Nile Virus — Project Description — City of Madison Heights
First, we continue to educate our residents about mosquitoes, their breeding cycle, their need for
a blood meal, West Nile Virus, and other mosquito borne arboviruses. Part of this education will
include what they can do to manage mosquitoes. Personal protection, mosquito avoidance,
repellants and drainage of standing water will be included. We will invite residents to call us if
they have been unable to talk a neighbor into draining stagnant water. Code Enforcement and/or
Sewer Division personnel will inspect such areas and solve concerned issues.
A second part of the education program will be to explain what the City is doing to control the
problem. This will include larviciding, surveillance, trapping, and ongoing analysis of the
success of the program. We will achieve these goals by utilizing the spring/ summer City
Newsletter and through the City's Web site and Cable messages.
The second part of the program will be the Larvicide program. We have completed larviciding
all publicly owned catch basins as well as larviciding 1,020 private catch basins in the City.
Every parking lot of every business was larvicided. Every pond in the City, both on City
property and Commercial and Industrial land was checked and larvicided with bulk Vectolex if it
held standing water. Those with fountains were left alone. Appropriate measures will be taken if
a larvae problem exists in water gate wells.
Third, we plan to monitor 12 catch basins throughout the spring and summer and all vernal
ponds at Friendship Woods and Madison Woods in addition to retention ponds. We will also
monitor two gate wells. These will be checked for the effectiveness of the program and
additional larvicide or other methods of larvae reduction will be used if required. These methods
may include draining an area, applying an approved oil to the surface of the water, or sound
waves to kill the larvae.
Fourth, we will trap adult mosquitoes, if necessary, to monitor the amount and type of mosquito
in a given area. Those trapped will be sent to either the Michigan Department of Agriculture or
Michigan State University for analysis.
We have asked our residents to notify us if someone in their household has been clinically
diagnosed with West Nile Virus.

The following is an excerpt from the minutes of a Regular Meeting of the
Madison Heights City Council of the City of Madison Heights, Oakland
County, Michigan held on Monday, July 12, 2004 at 7:30 p.m. Eastern
Daylight Time in the Municipal Building in said City.

204. Oakland County West Nile Virus Fund Program reimbursement
Motion by Councilman Clark,
Supported by Councilwoman Russell,
WHEREAS, upon the recommendation of the Oakland County Executive,
the Oakland County Board of Commissioners has established a West Nile
Virus Fund Program to assist Oakland County cities, villages and
townships in addressing mosquito control activities; and
WHEREAS, Oakland County's West Nile Virus Fund Program authorizes
Oakland County cities, villages and townships to apply for
reimbursement of eligible expenses incurred in connection with personal
mosquito protection measures/activity, mosquito habitat eradication,
mosquito larviciding or focused adult mosquito insecticide spraying in
designated community green areas; and
WHEREAS, The City of Madison Heights of Oakland County, Michigan
has incurred expenses in connection with mosquito control activities
believed to be eligible for reimbursement under Oakland County's West
Nile Virus Fund Program.
NOW, THEREFORE BE IT RESOLVED, the Mayor and City Council of
Madison Heights authorizes and directs its Deputy City Manager, Peter
J. Connors, as agent for the City of Madison Heights, in the manner and
to the extent provided under Oakland County Board of Commissioner
Miscellaneous Resolution #04120 to request reimbursement of eligible
mosquito control activity under Oakland County's West Nile Virus Fund
Program.

On May 6, 2004, the Oakland County Board of Commissioners, by way of Miscellaneous
Resolution #04120, adopted the West Nile Disbursement Policy for Fiscal Year 2004. The policy
will distribute $500,000 to the cities, villages and townships (CVT) of Oakland County to combat
the West Nile Virus.
The distribution for each CVT is based on a weighted formula of 75% for population and 25% for
total square miles. The amount designated for the City of Madison Heights is $ 10,744.01.
The project must meet the eligibility requirements as determined by the Oakland County Health
Division. The deadline for submitting your application for reimbursement from the Oakland County
West Nile Virus Fund is August 15, 2004.
Project applications must include: cover letter, project description and governing body resolution
(samples are attached). Invoices of actual costs are to be submitted once the work is completed.
All applications and eligible reimbursement requests are to be submitted to:

West Nile Virus Fund reimbursements are issued only after copies of paid invoices for eligible
expenses are submitted. In order to ensure processing of your reimbursement, please submit
