IN RE: WEST NILE VIRUS FUND REIMBURSEMENT FOR PROJECT IN THE CITY OF
WALLED LAKE - FISCAL YEAR 2005 ALLOCATION
TO THE OAKLAND COUNTY BOARD OF COMMISSIONERS
Chairperson, Ladies and Gentlemen:
WHEREAS the Oakland County Board of Commissioners via
Miscellaneous Resolution #05061 approved the extension of the West Nile
Virus Fund for FY 2005; and
WHEREAS the Oakland County Board of Commissioners established a
West Nile Virus Fund Disbursement Policy per Miscellaneous Resolution
#03060 in an effort to assist cities, villages and townships (CVTs)
with mosquito control measures to prevent the spread of West Nile Virus
and to help protect the health, safety and welfare of the people of
Oakland County; and
WHEREAS pursuant to the Disbursement Policy, the City of Walled
Lake has submitted a resolution of authorization to the County for
reimbursement of expenses incurred in connection with an eligible
mosquito control project; and
WHEREAS the FY 2005 authorized amount of funding for the City of
Walled Lake is 82,433.87 from the West Nile Virus Fund as repayment to
the City of Walled Lake for expenses incurred in connection with
mosquito control projects.
NOW THEREFORE BE IT RESOLVED that the Oakland County Board of
Commissioners approves the project submitted by the City of Walled Lake
as eligible for reimbursement from the West Nile Virus Fund.
BE IT FURTHER RESOLVED that the Board authorizes the FY 2005
appropriation in the amount of $2,433.87 from the West Nile Virus Fund
(Account #90-210000-23030-9098) to repay the City of Walled Lake for
expenses incurred in connection with the mosquito control project, once
proper invoices are presented.

Dear Mr. Soave:
I am in receipt of a West Nile Virus fund reimbursement request from the City of
Walled Lake dated July 25, 2005. After having reviewed their documentation, I
find them in partial compliance with Oakland County Board of Commissioners
Miscellaneous Resolution 05061 (they are currently missing expense invoices)
and in compliance with at least one of the four major categories for
reimbursement. This letter will, therefore, certify that all expenses submitted for
reimbursement are for qualifying mosquito control projects. Should you have any

Dear Mr. Miller:
RE: West Nile Virus Fund Reimbursement Request
Pursuant to Oakland County Board of Commissioners Miscellaneous Resolution (MR)
04120, the City of Walled Lake hereby requests reimbursement under Oakland County's
West Nile Virus Fund Program for expenses incurred in connection with an eligible
mosquito control activity by our City.
Attached in support of this request are the following:
1. Project description — See Attachment A
2. Governing Body Resolution — See Attachment B
The City of Walled Lake understands that the review and processing of this
reimbursement request will be governed by the provisions of MR 04120, and certifies
that all expenses submitted for reimbursement were incurred in connection with a
qualifying mosquito control project.
The City would like to thank the County officials for making these funds available to the
City to help protect our residents from the West Nile Virus.

APPENDIX A
1. The pesticides will be used for larval mosquito control in catch basins
and retention ponds located throughout the City of Walled Lake. As
necessary, we will also be utilizing the pesticide in roadside ditches.
2. The geographic location of the pesticide application lies only within the
City limits of Walled Lake, in the County of Oakland, in the State of
Michigan.
3. The specific pesticides being applied include Altosid XR and Vectolex
WSP.
4. Upon receipt of our Certification of Approval, one treatment of Altosid
XR will be applied to catch basins located throughout the City of
Walled Lake as needed. This pesticide is timed released and will be
effective in the catch basins throughout the summer season.
Retention ponds under the jurisdiction of the City will be treated not
more than once every thirty- (30) days with VectoLex WSP. These
treatments will take place through September 2005. Roadside ditches
will receive VectoLex WSP treatments, as necessary, depending upon
water drainage throughout the City. This will be monitored through
heavy rain periods. Material Safety Data Sheets have been attached
for your review.
5. One briquette of Altosid XR will be applied to each catch basin located
in the City as needed. One briquette is adequate for 200 square feet
and will last, under normal conditions, approximately 150 days. One
pouch of VectoLex WSP will cover 50 square feet. Depending upon
the square footage of the retention pond, the adequate number of
pouches will be applied every thirty- (30) days. In drainage ditches,
the same principal will apply. No dosages will exceed the
recommended manufacturer specifications.
6. Application will be completed by City of Walled Lake employees or by
contractors who specialize in mosquito abatement.
7. The majority of allocated funds will be used to inspect and vacuum, as
needed, the catch basins throughout the City of Walled Lake.

RESOLUTION
WHEREAS, upon the recommendation of the Oakland County Executive, the
Oakland County Board of Commissioners has established a West Nile Virus
Fund Program to assist Oakland County cities, villages and townships in
addressing mosquito control activities; and
WHEREAS Oakland County's West Nile Virus Fund program authorizes Oakland
County cities, villages and townships to apply for reimbursement of eligible
expenses incurred in connection with personal mosquito protection
measures/activity, mosquito habitat eradication, mosquito larviciding or focused
adult mosquito insecticide spraying in designated community green areas; and
WHEREAS the City of Walled Lake, Oakland County Michigan has incurred
expenses in connection with mosquito control activities believed to be eligible for
reimbursement under Oakland County's West Nile Virus Fund Program.
NOW THEREFORE BE IT RESOLVED that this Council authorizes and directs it
Director of Public Works as agent for the City of Walled Lake in the manner and
to the extent provided under Oakland County Board of Commissioner
Miscellaneous Resolution 04120, to request reimbursement of eligible mosquito
control activity under Oakland County's West Nile Virus Fund Program.

reviewed the above referenced resolution and finds:
1. Funding of $2,433.87 is available in the West Nile Virus
Fund for FY 2005 to reimburse the City of Walled Lake for
this project; no additional appropriation is required.
2. The Committee recommends adoption of this resolution.
