MISCELLANEOUS RESOLUTION 106018 February 2, 2006

IN RE: AREA AGENCY ON AGING — SUPPORT OF THE AREA AGENCY ON AGING 1-B'S
APPLICATION TO SEEK DESIGNATION AS THE SINGLE POINT OF ENTRY FOR LONG TERM
CARE IN OAKLAND COUNTY
To the Oakland County Board of Commissioners
Chairperson, Ladies and Gentlemen:
WHEREAS, according to the Southeast Michigan Council of Governments, Oakland County's age
65+ population is now increasing by almost 3,000 individuals each year; and
WHEREAS, an increasing number of these individuals require assistance in performing
necessary activities of daily living in order to maintain their independence; and
WHEREAS, in June, 2005 the Governor's Medicaid Long Term Care Task Force issued a report
calling for action to create an integrated system that appears seamless to the consumer, yet takes
advantage of the variation of long term care programs at the local, state, and federal levels"; and
WHEREAS, the report recommends that the Michigan Department of Community Health
"designate locally or regionally-based 'Single Point of Entry' agencies for consumers of long term care
and mandate that applicants for Medicaid funded long term care go through the Single Point of Entry
agency to apply for services"; and
WHEREAS, Executive Order # 2005 — 14 states that "by June 30, 2006, the Department shall
establish not less than three single point of entry demonstration programs for the delivery of long term
care supports and services", and intends to have single points of entry in place statewide within three
years; and
WHEREAS, the Michigan Department of Community Health has issued a request for proposals to
implement the first Single Point of Entry programs; and
WHEREAS, the Area Agency on Aging 1-B has served as an access point for older Oakland
County residents and adults with a disability seeking home and community-based long term care services
since the AAA 1-B Care Management program was initiated in 1982; and
WHEREAS, the Area Agency on Aging 1-B is Michigan's largest Medicaid MI Choice Home and
Community-Based Elderly/Disabled Waiver program provider and administers in-home service programs
utilizing federal, state, and United Way dollars; and
WHEREAS, the Area Agency on Aging 1-B has developed a state-of-the-art call center with
unlimited capacity to provide information and assistance to callers seeking information on long term care
resources and alternatives; and
WHEREAS, the Area Agency on Aging 1-B quality assurance programs have documented that in-
home service programs produce positive health outcomes which reduce health care utilization and save
consumer and public dollars; and
WHEREAS, the Area Agency on Aging Board of Directors has directed the agency to seek
designation as the Single Point of Entry for long term care for Oakland County, in addition to the other five
counties of its region; and
WHEREAS, the Oakland County Board of Commissioners has full confidence that the Area
Agency on Aging 1-B, serving as a Single Point of Entry that serves as an independent source of
information to area seniors, will work in collaboration with other public and private organizations to
develop a high quality service that is responsive to consumer needs.
NOW, THEREFORE BE IT RESOLVED that the Oakland County Board of Commissioners
supports the Area Agency on Aging 1-B's application to seek designation as the Single Point of Entry for
long term care in Oakland County.
Chairperson, on behalf of the General Government Committee, I move the adoption of the
foregoing resolution.

LONG TERM CARE SINGLE POINT OF ENTRY APPLICATION
SUMMARY WORKPLAN
Goal 1:
Long term care stakeholders will have ample opportunities to provide input into
the development and implementation of the AAA 1-B Long Term Care Single
Point of Entry
Objective 1.1:
Establish six local Consumer Advisory Boards with members from various Medicaid
long term care programs for the purpose of advising the AAA 1-B on matters related to
development and implementation of the Single Point of Entry
Objective 1.2:
Conduct a Regional Community Needs Assessment in collaboration with long term care
consumers and stakeholders to evaluate long term care services and establish outcome
indicators
Goal 2:
Long term care providers, consumers, and pathway entities will know about the
Single Point of Entry and utilize it as an access point for long term care
information and services
Objective 2.1:
Develop and implement a community education and outreach plan in collaboration with
local hospitals, nursing homes, Centers for Independent Living, disease-specific
organizations, and other long term care stakeholders, and the general public
Objective 2.2:
Develop specific outreach strategies for identified special populations with language,
cultural, or other barriers
Goal 3:
The AAA 1-B Single Point of Entry will perform a range of core functions that are
necessary to educate consumers about long term care options and facilitate
access to desired supports
Objective 3.1:
Provide information and preliminary screening for long term care benefits

Objective 3.2:
Implement person centered planning practices and protocols
Objective 3.3:
Develop protocols with collaborating community agencies that are pathway entities to
offer proactive long term care counseling on choices and benefits
Objective 3.4:
Develop collaboration protocols with local nursing facilities for referrals of potential
residents, and person centered planning and supports coordination for SPE referred
residents
Objective 3.5:
Develop transition services that allow consumers to move from one region, program, or
setting to another
Objective 3.6:
Develop a system of supports coordination based on person centered planning that will
provide functional screening and long term care options counseling, create supports
plans, authorize services, advocate for consumer-directed outcomes, and re-evaluate
consumer status and goals
Objective 3.7:
Develop collaboration protocols with local offices of the Michigan Department of Human
Services for financial eligibility determination and targeted collaboration for the most
vulnerable and/or highest cost consumers
Objective 3.8:
Develop collaboration protocols with county Community Mental Health Authorities to
assure the coordination of services for persons with concurrent mental health and long
term care needs
Goal 4:
Long term care consumers will be satisfied with their chosen array of long term
care supports and health outcomes
Objective 4.1:
Develop a Single Point of Entry quality management plan based on the Centers for
Medicare and Medicaid Services Framework for Quality in Home and Community-
Based Services

Goal 5:
Long term care stakeholders and the Michigan Department of Community Health
will know about progress in developing Single Point of Entry functions and
achieving long term care outcomes
Objective 5.1:
Develop an annual reporting plan that includes information on regional long term care
access, the development and operation of core functions and methods, performance
indicators, and consumer outcomes
Objective 5.2:
Develop AAA 1-B and vendor capacity to meet Michigan Department of Community
Health requirements for electronic information system screening, assessment, tracking,
outcome analysis, and bill submission
