1. DEPARTMENT OF HEALTH AND HUMAN SERVICES/HEALTH DIVISION - OAKLAND
SCHOOLS, HOME VISITATION GRANT ACCEPTANCE
The Health Division is requesting acceptance of the Home Visitation Grant in the amount of $126,106,
and to create one (1 ) Special Revenue (SR), Full-Time Eligible (FTE) Public Health Nurse III position. The
Michigan Department of Education awarded this grant to Oakland Schools, who intends to sub-award it to
Oakland County. Great Start Collaborative did an assessment to determine how they may improve school
readiness and identified Nurse Family Partnership as an evidence -based model that is utilized for infant
mortality, child abuse prevention, and has evidence that school readiness is improved. Enough funding
has been provided to expand the Nurse Family Partnership Program and to fund the Public Health Nurse
III position. The intention is to utilize this nurse to do Nurse Family Pr ogram activities in the southern
portion of the County, such as Oak Park, Southfield, Madison H eights, and Hazel Park. This will hopefully
