Health & Human Services - Health Division - Resolution Authorize Waiving of Polio

Chairperson and Members of the Board:
WHEREAS Polio is a disabling or life-threatening disease caused by poliovirus, which has been
eliminated in the United States since 1979; and

prevent cases of polio and educates residents on the importance of polio vaccination; and
WHEREAS OCHD is requesting approval to waive the $40 polio vaccination fee and associated $7
