ITEM CATEGORY SPONSORED BY Contract Penny Luebs INTRODUCTION AND BACKGROUND The Oakland County Sheriff’s Office is amending the FY 2021 Interlocal Agreement with the Oakland County Health Network (OCHN) for the Medication-Assisted Treatment (MAT) Program and the Jail Alliance with Support (JAWS) Program to expand the medication and personnel services offered to both the in-county and out-of -county inmates eligible for the MAT program. The purpose of the amendment is to increase the accessibility of MAT and decrease overdose prevalence by providing all eligible inmates at the Oakland County Jail with access to the full array of MAT services. Oakland County will pay OCHN for the following services: 1) nursing staff to perform lab testing, drug testing, distribution of oral medication and injections, and monitoring of symptoms of MAT eligible participants 2) a physician to perform medical evaluations and medication reviews for MAT eligible participants 3) behavioral health staff to perform individualized therapy based on individually assessed needs at a minimum of bi-weekly and group therapy at a minimum of weekly 4) administrative costs 5) increased doses of MAT medication Methadone, Vivitrol, and Suboxone 6) a Peer Recovery Coach to provide support to eligible MAT participants during their housing at the jail, during the release process from the jail and until they have attended their first appointment for community-based MAT treatment services 7) MAT medication Sublocade injections BUDGET AMENDMENT REQUIRED: Yes Committee members can contact Michael Andrews, Policy and Fiscal Analysis Supervisor at 248.425.5572 or andrewsmb@oakgov.com, or the department contact persons listed for additional information. This Amendment #4 shall take effect on the date when fully executed by the parties and extend through date of September 30, 2023, or is terminated by any of the Parties pursuant to section 17 of the Contract. In consideration of the mutual promises, representations, assurances, agreements and provisions in the Agreement and this Amendment the adequacy of which is hereby acknowledged by the Parties, the Parties agree to amend the contract as follows: 1. The attached amended attached Exhibit III shall replace the original Exhibit III. 2. The attached Exhibit V shall be added to the Agreement which expands the medication and personnel services offered to the inmates eligible for the MAT program. 3. Except as modified herein, all other terms of the Agreement remain in full force and effect through the date of September 30, 2023. For and in consideration of the mutual assurances, promises, acknowledgments, warrants, representations and agreements set forth in the Agreement and this Amendment, and for other FY2023 MAT and JAWS PROGRAM AGREEMENT AMENDMENT #4 good and valuable consideration, the receipt and adequacy of which is hereby acknowledge, the undersigned hereby execute this Amendment 4 on behalf of the Parties, and by doing so legally obligate and bind the parties to the terms and conditions of this Amendment 4. BOUCHARD BY: _____________________________________ DATE: _______________________ Oakland County Sheriff FY2023 MAT and JAWS PROGRAM AGREEMENT AMENDMENT #4 Amended Exhibit III FUNDING FOR INMATE CASEWORK SPECIALIST POSITION 1) OCHN shall fund the County in an NOT TO EXCEED (NTE) amount for the following position: 1) OCHN shall fund the County in a NOT TO EXCEED (NTE) amount for the following position: one (1) full-time eligible Inmate Casework Specialist, including fringe benefits, in the amount of $89,104.00 for services from October 1, 2020 through September 30, 2021, $44,552.00 for services from October 1, 2021 through March 31, 2022, $45,633.50 for services from April 1, 2022 through September 30, 2022, and $99,330.00 for services from October 1, 2022 through September 30, 2023. 2) To ensure timely payment, the County shall invoice OCHN for the full amount and direct the invoice to the attention of OCHN’s Chief Financial Officer. Prior to the close of FY2023, the County will provide a cost settlement breakdown of the funding to OCHN’s Chief Financial Officer twenty (20) days after the end of the fiscal year. Cost settlement of this agreement will be based upon a comparison of the actual allowable expense for the Inmate Casework Specialist position and the amount paid by OCHN to the County. Any unspent funds will be returned by the County to OCHN within 30 days of the final settlement letter. 3) The Inmate Casework Specialist shall be assigned duties related to the MAT Program for eligible in-county inmates and eligible out-of-county inmates. 4) The County will track and collect the following data, which it will provide to OCHN on a quarterly basis: a. Amount of time the Inmate Casework Specialist spends on MAT Program related duties for in-county inmates. b. Amount of time the Inmate Casework Specialist spends on MAT Program related duties for out-of-county inmates. FY2023 MAT and JAWS PROGRAM AGREEMENT AMENDMENT #4 Exhibit V FUNDING FOR INCREASED ACCESSIBILITY TO MEDICATION-ASSISTED TREATMENT TO ELIGIBLE IN- COUNTY AND OUT-OF-COUNTY OCJ INMATES Oakland Community Health Network (OCHN) is responding to Oakland County’s request to provide Oakland County Jail (OCJ) Medication Assisted Treatment (MAT) to both in county and out of county individuals who come into the OCJ who are currently being prescribed an Opiate Use Disorder (OUD) medication or wish to be initiated onto an OUD medication. The purpose of this Exhibit is to increase the accessibility of MAT and decrease overdose prevalence by providing all eligible OCJ inmates with access to the full array of MAT services. 1. In consideration of all the above factors, County shall fund OCHN in a NOT-TO EXCEED (NTE) amount of $478,060 for the following budget items: a) OCHN Contractor Sacred Heart and Wellness nursing staff will perform lab testing, drug testing, distribution of oral medication and injections, and monitoring of symptoms of MAT eligible participants at an annual NTE cost of $90,000 and monthly billing rate of $7,500; b) OCHN Contractor Sacred Heart and Wellness physician will perform medical evaluations and medication reviews for MAT eligible participants at an annual NTE cost of $114,000 and monthly billing rate of 9,500; c) OCHN Contractor Sacred Heart and Wellness behavioral therapy staff will perform individualized therapy based on individually assessed needs at a minimum of bi-weekly and group therapy at a minimum of weekly at an annual NTE cost of $60,000 and a monthly billing rate of $5,000; d) OCHN Contractor Sacred Heart and Wellness administrative costs at an annual NTE cost of $6,000 and monthly billing rate of $500; e) Increased doses of MAT medication Methadone, Vivitrol, and Suboxone at an annual NTE cost of $24,000 and a monthly billing rate of $2,000; f) OCHN Contractor Peer Recovery Coach will provide support to eligible MAT participants during their housing at the OCJ, during their release process from the OCJ, and until they have attended their first appointment for community based MAT treatment services. Peer Recovery Specialist services will be provided at an annual NTE cost of $32,000 and monthly billing rate of $2,667; FY2023 MAT and JAWS PROGRAM AGREEMENT AMENDMENT #4 g) MAT medication Sublocade injections under the following NTE conditions: 1) each Sublocade injection at a NTE cost of $1411; and 2) total FY2023 not-to-exceed cost of $125,000. h) OCHN Administration of Sacred Heart and Wellness Contract fee at an annual NTE cost of $27,060 and monthly billing rate of $2,255. 2. Under mutual agreement of the parties, the NTE funds within each budget line item contained in Sec. 1(a) through 1(h) may be moved to other budget line items, as long as the total NTE does not exceed $478,060. 3. To ensure timely payment, OCHN will invoice the County quarterly for the services provided by the OCHN Contracted personnel. OCHN will calculate this amount based on the costs of each position which shall not to exceed $478,060 for the Grant Fiscal Year.
