INTRODUCTION AND BACKGROUND
Oakland Schools was awarded continued funding by the Michigan Department of Education for their
Home Visitation Grant in the amount of $40,477 for the period of October 1, 2023 through
September 30, 2024. Oakland Schools is continuing the sub-award with Oakland County under the
Home Visitation Grant for the expansion of the Nurse Family Partnership program in the southeast
region of Oakland County. The grant funds will provide evidenced-based model fidelity home
visitation/nurse family partnerships in the southeast region of Oakland County, including Hazel Park,
Madison Heights, Oak Park and Southfield, to families who are economically disadvantaged.
The funding will partially support one (1) Special Revenue (SR) Full-Time Eligible (FTE) Public
Health Nurse III position and the remaining position costs are offset by funding awarded through the
Michigan Department of Health and Human Services (MDHHS) FY 2024 Emerging Threats Local
Health Department American Rescue Plan grant project.
There is no local grant match.
This grant agreement has completed the Grant Review Process in accordance with the Grants
Policy.
POLICY ANALYSIS
The acceptance of this grant does not obligate the County to any future commitment and
continuation of the Special Revenue position in the grant is contingent upon continued future levels
of grant funding.
