Appointment to the Oakland Community Health Network Board

Chair, the Board of Commissioners recommends the following Board action:
to appoint Sarah Guadalupe, as a General Public member, to the Oakland Community Health
Network for the remainder of the three-year term beginning September 5, 2024 and ending March 31,
2026.

Explain Why You Would Like To Serve On The Oakland Community Health Network:
Having been born, raised, and spending most of my life as a resident of Oakland County, I find that
this opportunity would allow me to give back to my community. My previous experience on the
Board, coupled with over 20-years of experience as a practicing social worker in the community would
allow me to have an understanding of the requirements and expectations of serving on the Board,
while continuing to advocate for and support the various constituencies, particularly those being
served.

Please Detail Any Prior Work-Related or Community Service Experience and Skills That May Contribute
To the Position You Are Seeking:
Through my education as a social worker, I have extensive work experiences and skills that I believe
would contribute to serving on the Board. I have worked in inpatient and outpatient mental health
agencies, emergency departments and crisis intervention centers, in K-12 educational setting, and as a
professor and administrator in higher education. I have been fortunate to hold several leadership
roles in many of these positions. Through these leadership roles, I have been able to demonstrate
strong advocacy skills, critical thinking skills, the ability to make data driven decisions, and effective
listening, communication and relationship building skills.
In my previous experience serving on the OCHN Board, I was elected by the Board to serve as the Vice-
Chair in November 2020 and Chair of the Search Committee for the CEO position (2020). Additionally, I
served as a Delegate to the Executive Board for the Community Mental Health Association of
Michigan.
I believe my work experience and previous experience on the Board are an asset and prepare me to
be successful in serving once again.
