Motion to create one (1) FTE Special Revenue Community Health Worker position in the Health
Infant Promotion Unit; further, authorize the Chair of the Board of Commissioners to execute this
agreement after final review by Risk Management and Corporation Counsel; further, recognize that
the budget amendment for the creation of this position will be included in the subsequent
Management and Budget quarterly forecast report.

INTRODUCTION AND BACKGROUND
Oakland County is adopting the evidenced based program Transition to Success® (TTS) that
screens for Social Determinants of Health (SDOH) and provides assistance with achieving goals to
resolve inequities. This program is one of the first evidence-based standard of care to treat the
condition of poverty as an environmentally based medical condition, integrating uniform protocols
and analytics to directly respond to the social determinants of health. TTS protocols create a
coordinated system of care across healthcare, human service, government, education, and faith-
based organizations.
TTS trained direct care staff work with clients to prioritize and ensure access to all available services
and supports across their community. Oakland County Health Division requests to use Maternal
Child Health grant funds accepted via resolution 2024-4440 from the Michigan Department of Health
and Humans Services to staff one Special Revenue Community Health Worker to implement TTS
within the maternal home visiting program.
POLICY ANALYSIS
On September 19, 2024 resolution #2024-4440 allowed for the acceptance of the FY 2025 Local
Health Department Comprehensive Agreement in the amount of $16,922,160. A portion of these
funds were allocated to the Maternal Child Health Program within Oakland County. This request is
to create a new Special Revenue FTE Community Health Worker position, in the Health Infant
Promotion Unit, to assist the Oakland County Maternal Home Visiting Pilot Program, which will be
supported by the Maternal Child Health Program Funds.
Amendment #1 was recorded as a Receive and File item on the December 3rd, 2024 PH&S
agenda, it was less than 15% and was completed administratively.

OVERVIEW:
The Oakland County Health Division is requesting an Amendment to the FY 2025 Local Health Department
Comprehensive Agreement. The original agreement totaling $16,922,160 was adopted by the Board of
Commissioners on September 19,2024, via Miscellaneous Resolution #2024-4440.
