ITEM CATEGORY SPONSORED BY Grant Penny Luebs INTRODUCTION AND BACKGROUND The Michigan Public Health Institute awarded the Oakland County Health Division funding through the Michigan Department of Health and Human Services 2024 Regional Health Equity Council Backbone Organization grant program to support and maintain membership of the Regional Health Equity Council, assist in developing and implementing an action plan to address and reduce causes of health inequities in the region, and implement practices and policies to reduce health disparities and improve health outcomes. The original Subrecipient Grant Agreement was adopted by the Board of Commissioners on December 7, 2023 via Miscellaneous Resolution #2023-3588. The Oakland County Health Division awarded a portion of the grant funding in the amount of $200,300 to the Pontiac Community Foundation for administrative support in overseeing payments to council community member representatives and other organizational members and a mini-grant program through May 31, 2024. The Oakland County Health Division has drafted Amendment #1 to the Subrecipient Agreement with the Pontiac Community Foundation to extend the end date from May 31, 2024, to September 30, 2024 and increase the funding from $200,300 to $202,300. The Pontiac Community Foundation has agreed to the terms included within the Subrecipient Agreement.
