authorize the Chair of the Board of Commissioners ot execute the attached agreement; further,
amend the FY 2026 budget as detailed in the attached Schedule A.

INTRODUCTION AND BACKGROUND
The Health Division has received an Amendment to the Interlocal Agreement between Community
Health Network (OCHN) and Oakland County for Substance Use Disorder Prevention Services. The
Amendment changes the contract number of the Agreement from 2022-0215-SUDP to 2022-1181
and adds funding in the amount of $180,178 to carry out Fiscal Year 2026 work plan objectives for
substance use disorder prevention services for the period October 1, 2025 through September 30,
2026.
Fiscal Year 2026 work plan objectives for substance use disorder prevention services include the
following:
• Providing evidence-based substance use prevention education to middle school youth using
the tobacco and vaping program CATCH My Breath to schools and community
organizations.
• Providing vaping and tobacco prevention education to adults who interact with youth using
the Michigan Department Health & Human Services presentation.
• Providing technical assistance to the older adult population to increase awareness and
opportunity to safely dispose of medication.
The Substance Use Disorder Prevention Services funding of $180,178, which consists of $163,798
in grant funding and a local in-kind match of $16,380. The in-kind match is budgeted in the Health
Division Health Promotion Services General Fund Budget and accounts for salaries, personal
mileage, telephone communications, and IT operations. Further, the grant funding is sufficient to
continue two (2) Special Revenue (SR) Full-Time Eligible (FTE) Public Health Educator III positions
(1060261-00960 and #1060261-07497).
POLICY ANALYSIS
• The original agreement for the Substance Use Disorder Prevention services with Oakland
Community Health Network was approved via MR #2021-502 on December 9, 2021.
• Amendment #1 was approved by the Administration on May 27, 2022, because it was under
15% variance from the current award and did not require personnel changes.
• Amendment #2 was approved via MR #2022-419 on December 8, 2022.
• Amendment #3 was approved via MR #2023-3503 on November 16, 2023.
• Amendment #4 by the Administration on June 12, 2024, because it was under the 15%
variance from the current award and did not require personnel changes.
• Amendment #5 was approved via resolution #2025-5220 in the amount of $246,155. This
resolution noted that Amendment #6 would be forthcoming to deduct ARPA funding, which
was lost during FY 2025 federal and state budget cuts.
• Grant funding continues two (2) FTE positions; no HR action is needed.

The Parties, Oakland County ("County") and Oakland Community Health Network
("OCHN"), agree and acknowledge that the purpose of this Amendment is to modify as
provided herein and otherwise continue the present contractual relationship between the
Parties as described in their current Substance Use Disorder Prevention Services,
("Contract").
In consideration of the mutual promises, representations, assurances, agreements, and
provisions in the Contract and this Amendment, the adequacy of which is hereby
acknowledged by the Parties, the Parties agree to amend the Contract as follows:
1. Effective as of the date of this Amendment, the Contract Number of the Agreement
shall be changed from 2022-0215-SUDP to 2022-1181.
2. The Parties agree to extend the Agreement through September 30, 2026.
3. The Parties acknowledge and agree to FY26 Exhibit I: Financial and Reporting
Obligations, as attached hereto.
4. The Parties acknowledge and agree to FY26 Attachment B – Management by
Objectives in Exhibit IV, as attached hereto.
5. The Parties acknowledge and agree to FY26 Attachment C – Purchase of Services
Policies for Providers of Prevention Services in Exhibit IV, as attached hereto.

For and in consideration of the mutual assurances, promises, acknowledgments,
warrants, representations and agreements set forth in the Contract and this Amendment,
and for other good and valuable consideration, the receipt and adequacy of which is
hereby acknowledged, the undersigned hereby execute this Amendment on behalf of
the OCHN and County and by doing so legally obligate and bind the OCHN and
County to the terms and conditions of the Contract and this Amendment:

The County shall submit an invoice to the OCHN on a monthly basis, which shall itemize all
amounts due and or owing by OCHN. The monthly invoice must reflect total actual program
expenditures by category. The invoices shall be submitted in a form and format agreed upon by
the Parties.
