Motion to approve amendment #1 to the Oakland Schools FY 2025 Home Visitation Grant Program
in the amount of $61,255 for the period of October 1, 2025, through June 30, 2026; further, delete
one (1) vacant Part-Time Non-Eligible 1,000-hour Public Health Nurse III, and one (1) vacant Part-
Time Non-Eligible 1,000-hour Auxiliary Health Clerk for the Nurse Family Partnership Program in
the Nursing Services Health Infant Promotion Unit (#1060291); further, authorize the Chair of the
Board of Commissioners to execute the agreement; further, amend the FY 2025 budget as detailed
in the attached Schedule A.

INTRODUCTION AND BACKGROUND
The Oakland County Health Division received Amendment #1 to the Oakland Schools FY25 Home
Visitation grant Agreement. The Amendment reflects a funding level reduction in the FY 2025 grant
award amount from $199,838 to $144,218, a total reduction of $55,620, to reflect the actual
amounts spent for the FY 2025 grant period.
Amendment #1 also adds FY 25 carryover funding in the amount of $61,255 for the period October
1, 2025 through June 30, 2026 for continued support in the provision of evidenced-based model
fidelity home visitation/nurse family partnerships in the southeast region of Oakland County,
including Hazel Park, Madison Heights, Oak Park and Southfield, to families who are economically
disadvantaged:
Due to changes in the grant funding, it is requested to delete one (1) vacant Special Revenue Part-
Time Non-Eligible (PTNE) 1,000 hours per year Public Health Nurse III position (P00017208), and
one (1) vacant SR PTNE 1,000 hours per year Auxiliary Health Clerk position (P00017209) for the
Nurse Family Partnership Program in the Nursing Services Health Infant Promotion Unit
(#1060291).
POLICY ANALYSIS
• The subaward provides funding for the provision of evidenced-based model fidelity home
visitation/nurse family partnerships in the Southeast region of Oakland County, including
Hazel Park, Madison Heights, Oak Park and Southfield, to families who are economically
disadvantaged.
• Grant funding was accepted via resolution #2024-4640, in the amount of $350,000.
o This included (1) FY 2024 carryover funds of $150,162, which needed to be spent by
June 30, 2025 and (2) FY 2025 funds of $199,838, which needed to be spent by
September 30, 2025.
o Acceptance of grant funding also authorized the creation of one (1) FTE position and
two (2) PTNE positions.
• Amendment #1 requests the deletion of two (2) PTNE positions; HR action will be needed.
There is a total of three (3) positions associated with this grant. The remaining one (1) FTE
position will remain on the grant until funding levels for the 2027 FY are determined. As it is
unknown at this time if this program will continue into the 2027 Fiscal Year. Oakland Schools
is no longer able to support this program due to continued funding cuts.
• The remaining funding of $61,255 for this grant agreement must be spent by June 30, 2026.
• Per the department, the personnel that was paid through this grant funding were only partially
supported through the Home Visitation Grant. The Oakland County HHS department must
now supplement 100% of the salary costs through another grant. The department has the
funding to do so.
